Retrospective Analysis of405Patients with Aortic Dissection Combined Renal CystObjective:To analyze retrospectively of405patients with aortic dissection (AD), focusing on the cases combined renal cysts in order to understand the correlation between AD and renal cysts.Methods:Retrospective analysis of405clinical cases of AD, with emphasis on those combined renal cysts, from January2009to December2011in Second Xiangya Hospital of Central South University.1000patients with essential hypertension from examination center in a stratified and random way were selected as the hypertension control group and1000cases of normal subjects served as normal control group to analyze. And propensity-matched analysis among AD group, hypertension control group and normal control group were done after incorporating into the gender, diabetes, liver and kidney function and other factorsResults:1.328cases (81%) of the405AD patients have history of hypertension. Most of the Type-A AD patients have pain in chest and back while combining rate in Type-B AD is slightly less, meanwhile, the combining rate of lumbar pain symptom in the two types was not statistically significant (P>0.05). Symptoms as shortness of breath, limb weakness, nausea, vomiting, syncope, chest tightness, dizziness, heart palpitations and pericardial effusion were more likely to happen in Type-A AD than Type-B AD, the difference was statistically significant (P<0.05). But symptom of sweating and sign of pleural effusion has no significant difference (P>0.05).2. Among the405patients of AD group,121of341male cases (35.5%) and27of64female cases (42.2%) were detected renal cysts, the difference was not statistically significant (P>0.05);47of171Type-A AD cases (27.5%) and101of234Type-B AD cases (43.2%) were combined with renal cyst, the detection rate of renal cyst in Type-B AD cases is higher than that in Type-A AD cases(P<0.05).3. After the propensity-match between AD group and normal control group,364cases were found in AD group with161cases of Type-A AD (44.3%) and203cases of Type-B AD (55.7%).135cases of renal cyst were detected in the AD group (37.1%) which is significantly higher than that of the normal control group (49cases,9.0%)(P<0.05).4. After the propensity-match between AD group and hypertension control group,308cases were found in AD group with143cases of Type-A AD (46.4%) and165cases of Type-B AD (53.6%).107cases of renal cyst were detected in the AD group (34.7%) which is significantly higher than that of the hypertension control group (76cases,16.5%)(P <0.05).Conclusion:(1) AD is often associated with renal cysts. Type-B is more common than Type-A without gender differences.(2)Pain in chest and back is more common in Type-A than Type-B while the merger rate of lumbar pain has no significant difference. The significance of PC1/PC2expression in blood vessel walls of type A aortic dissectionObjective:To investigate the expression of the PC on the aortic wall of aortic dissection(AD) patient, and to explore the relationship between PC expression and AD.Methods:10specimens were taken from patients with aortic dissection, who were admitted to our hospital due to the treatment of hypertension and type A acute aortic dissection surgery from April2011to December2011. The specimens were taken from1cm edge of the excision of the ascending aorta from the dissection of cardiac surgery patients.7males and3females, aged from39to65years were included in the AD group.4specimens in the control group were obtained from the ascending aortic wall in aortic valve replacement surgery from patients with valvular heart disease in the same period.3males and1female, age from52to67years old with no history of hypertension were included in the control group. The expression level of PC1and PC2were detected by Western blot.Results:PC1protein expression level was0.649±0.557in the tissue of the aortic wall from the AD group, which was significantly lower than the normal control group0.920±0.091(P<0.05), PC2protein expression level in the AD group was0.395±0.075, which was significantly lower than the normal control group0.814±0.062 (P<0.05)Conclusion:The onset of type A AD may be associated with the deficiency or lower expression of PC.
