Abstract/Summary:
Object ive:Based on the clinical analysis of the patients with aortic dissection, improve doctors’understanding of the AD, improve the rate of early diagnosis, reduce the misdiagnosis and mortality, quickly identify critically ill patients; and study the character of TCM Syndromes, to provide the basis for differentiation of TCM.Methods:68cases of AD patients in the First Hospital of the first hospital affiliated to Guangzhou University of Chinese Medicine from January2003to March2012were analyzed retrospectively. We analysed the ages, genders, risk factors.clinical manifestations.examinations, diagnosis, treatments of all patients and Tongue pulse. SPSS12.0software was used to analysis the Result to summarize AD risk factors, clinical manifestations, prognosis and its character of TCM syndromes.Results:Among the68patients,56patients had hypertension(82.35%),7patients(12.5%)hadn’t been treated by medicine and31patients’(55.36%)blood pressure hadn’t been controlled well. Hypertension group (group A) and non-hypertensive group (Group B), Blood pressure standards group (Al) and blood pressure hadn’t been controlled well group(A2), and the difference Between the their Systolic blood pressure was significant(P<O.05). The cl inical manifestations of AD were complex and diverse, but the main clinical manifestations were pain(92.65%), Chest pain43cases (63.24%), Shoulder pain25cases (36.76%), Abdominal pain23cases (33.82%), Waist and Leg pain cases8(11.76%) among the68patients. Elements of basis syndromes of AD distribution:18qi deficiency(26.14%),2blood deficiency (2.94%),8yin deficiency (11.76%),3Yang deficiency (4.41%),39blood stasis (57.35%),10qi stagnation (14.7%),1cold coagulation (1.47%),7damp pathogen (10.29%),11damp-heat (16.18%),14turbid phlegm (20.59%),15phlegm-heat (22.06%),2excessive fluid (2.94%). The AD was mainly classed into5types, turbid phlegm and blood stasis syndrome, phlegm-damp syndrome, qi stagnation and blood stasis syndrome qi deficiency and blood stasis syndrome, deficiency of both qi and yin.Conclusion:Hypertension is a very important causation and risk factor of Aortic dissection, which will effect the development and prognosis of Aortic dissection. Pain is the main clinical manifestations of aortic dissection, Pay attention to the diagnosis and differential diagnosis of pain. The TCM basic syndromes elements of Aortic dissection are qi deficiency, Yin deficiency, qi stagnation, Stasis and Phlegm. Among them, phlegm and stasisbecome the core pathology of the disease. Empirical is more common than deficiency in typing. Usually, Empirical expresses as stasis syndrome and phlegm, while deficiency mainly expresses as deficiency of both qi and yin.
