Objective: Explore the clinical manifestations and pathological features ofmalignant ovarian endometriosis cysts. In order to improve early diagnosis andtreatment.Methods:Retrospective analysis2002.01.01-2012.09.30Dalian City MaternityHospital during surgery. Pathologically confirmed2,253cases of ovarian endometriosiscysts,of12cases of patients with malignant. Analysis of the relationship between age,menopause status, clinical presentation, preoperative tumor marker examination,laboratory examinations, histological type, staging and prognosis factors and malignanttransformation.Results:The malignant ovarian endometrial cyst patients with an average age of46.6years,The median age was48years. premenopausal9cases (75%),menopause in3cases (25%),12patients with clinical symptoms of pelvic mass in12cases (100%).accompanied by lower abdominal pain, low back pain symptoms of12cases (100%).Dysmenorrhea performance significantly in6patients (50%), No significantdysmenorrhea performance in6patients (50%). Vaginal bleeding manifestations0(0%),the mass rapid increase of time of less than2months in11cases (91.7%),Bultrasound examination revealed a cyst diameter greater than8cm in10cases (83.3%),Mass in the left side in8cases (66.6%). The right side in2cases (16.7%), bilateral in2cases (16.7%). CA125values in the normal range (<35U/ml)8cases (66.7%),3patients(25%) between35U/ml and70U/ml, greater than70U/ml by one cases (8.3%).B-shownin the nature of mass cystic7cases (58.3%), nature is mixed in3cases (25%), thenature of the solid in2cases (16.7%). Clear cell carcinoma and endometrioid carcinomais a common tumor tissue type. Malignant clear cell carcinoma in9cases (75%), and malignant endometrial carcinoma in2cases (16.7%), the malignant serousadenocarcinoma in1case (8.3%). The clinical stage of Phase I in10cases (83.3%), IIPhase2cases (16.7%). Simple myoma5cases (41.7%), simple merge adenomyosis in2cases (16.7%), uterine fibroids and merge adenomyosis in1case (8.3%).12patientswith a mean follow-up time of47.8months (5-115months),5cases were lost, and theremaining seven patients were alive, the longest survival time was8years and1month (97months).Conclusion: Malignant ovarian endometriosis patients over the ageperimenopausal, risk of endometriosis history of malignant. The clinical manifestationsof the recent increase in pelvic mass increase more than two months, up to8cm indiameter, and with significant lumbar pain. CA125values in the normal range, historyof estrogen use, or uterine fibroids, adenomyosis, endometrial cancer patients withmalignant risk. Malignant type of clear cell carcinoma and endometrioid carcinoma,clinical stage I, II, better prognosis. Clinically, the above clinical features of ovarianendometriosis cysts should be alert to the risk of malignant transformation.
