To Explore Clinical Efficacy Of Ulinastatin On Patients With Septic Shock

Objective:To explore clinical efficacy of ulinastatin on patients with septic shock.Method:50patients with septic shock were randomly divided into conventional doses ulinastatin group (conventional-dose group,25cases) and super large doses ulinastatin group (super large doses group,25cases). Conventional-dose group were intravenously injected200,000U of ulinastatin every time,2times a day. While the super large doses group were cured one million U half an hour by rapid intravenous injection,5million U ulinastatin within5hours. Routine treatment were carried out the next day day,200000U each time,2times a day. APACHEII ratings score, serum ALT, AST and lactic acid content of Id,3d and5d were measured after treatment with drugs. We respectively measured two groups’urine output, heart rate and systolic blood pressure of1n,4h,12h, Id and3d after treatment with drugs. We recorded the time of shock recovery, the hospitalization days and the time return to a stable condition.Results:①After treatment, the APACHEII score, ALT, AST, Lac each index values of1d,3d,5d in high-dose treatment group were lower than those of conventional control group;②The urine output detection value of1h,4h,12h,24h,72h in high-dose treatment group were all higher than those of the normal control group after dosing; The heart rate detection value of1h,4h,12h,24h,72h in high-dose treatment group were all lower than those of the normal control group by using drugs; The systolic blood pressure detection value of1h,4h,12h,24h,72h in high-dose treatment group were all higher than those of the normal control group after medication.③The shock recovery time of high-dose treatment group was significantly shorter than normal control one; The hospitalization days in intensive care unit was obviously less than normal control one.④The mortality:17people died in the conventional group, the fatality rate was68percent; nine people died in the high-dose treatment group, the fatality rate was36 percent.Conclusions:Large doses Ulinastatin can increased urine output, also makes systolic blood pressure significantly higher, heart rate distinctly decreased of patients with septic shock. The blood levels of ALT, AST and Lac are all controlled. Large doses Ulinastatin can reduce mortality, control septic shock patient’s condition and improve prognosis.
