Treatment Of Auricle Keloid By V-shape Separative Surgery

Objective: Provide a simple surgery for the treatment of auricle keloid.Methods: From January2012to April2013，6patients,7side ears(5unilateral and1bilateral ear)with keloid at the aural region. There were all females，aged from22to28years．The course of disease ranged from1year to3years．The shortest time of theappearance of auricle keloid was1month after the earhole piercing,and the longest was4months. The causes of disease was earhole piercing. All of them didn’t accept anytreatment before coming. The sizes of keloids ranged from0.4cm×0.4cm×0.5cm to3.0cm×2.5cm×2.0cm. With globular、nodular and dumb-bell shape.The keloids werehard and the color were dark red.All the patients had worn earrings made of metal orplastic or the tea bar after earhole piercing. Three ears had inflammation in one weekafter the piercing, and two of them had serous fluid seepaged from the earhole, the otherhad pus exudes. These symptoms disappeared after they take out the earrings. Eliminatesurgical contraindication, all the auricle keloids were treated by V-shape separativesurgery with local anesthesia, without any adjuvant therapy.All the keloid were sent toathological examination. All the patients were followed up over12months to14months after the surgery, and the treatment effect was evaluated by the Cai J L[1]3levelevaluation criteria. The recent relevant literatures about keloid formation in pathologyand physiology were extensively investigated and were comprehensive analyzed, andcombined with the pathological results to investigate the feasibility and the effect ofV-shape separative surgery of auricle keloid.Results: During the follow up over12months to14months, there were nosignificant scar hyperplasia and the outlook of the auricles were satisfying. Pathologicalresults of auricle keloid showed increased collagen fibers were arranged disordered,and accompanied by a small amount of white blood cells, lymphocytes and fibroblasts.Among them there were blood vessels grown, but in the Cutting edge of skin andcartilage, there wasn’t abnormal pathological appearance and much collagen. Conclusion: It is effective to treat auricle keloid by the V-shape separativesurgery.It is an ideal surgical treatment. The focal pathogenesis of auricle keloid becametheoretical basis of the V-shape separative surgical treatment of auricle keloid.
