Explore the different age groups, differences in clinical characteristics inpatients with spontaneous subarachnoid hemorrhage;2. Study the impact of prognostic factors in patients with subarachnoidhemorrhage. Methods:Retrospective analysis of2002~2012the Second Affiliated Hospital ofDalian Medical University,Department of Neurology and Neurosurgery370confirmedcases of spontaneous SAH cases of hospitalized patients,male164,female206cases,male:female=1:1.26.18~44years old youth group;45~59for the middle-agedgroup;aged60and over for the older group.Chi-square test was used to compare theincidence of various age groups etiology, clinical manifestations, acute complications, the results of the CT and cerebral angiography, treatment whether there are differences, inspection standards set at0.05,P value <0.05,the difference was statisticallysignificance.Prognosis GOS score5cases were included in the clinical outcomesatisfaction group, the score of1~4cases of clinical outcomes are not satisfied group,Single factor chi-square test and multivariate logistic regression model analysis ofprognostic factors,Backward-LR method,Inspection standards at0.05,exclusioncriteria were set at0.1.According to the partial regression coefficients for each factor, the standard error of the partial regression coefficients, odds ratio, odds ratio95%confidence interval later identified with clinical risk factors associated with satisfaction,P <0.05,the difference was statistically significant.Results:Etiology:aneurysm195cases (52.70%),Middle-aged group of45~59is the most common, followed by hypertensive arteriosclerosis, common in the elderlygroup.Clinical manifestations:The elderly group headache, nausea and vomiting andthe incidence of language barriers with the middle-aged group were significantlydifferent; The incidence of middle-aged patients with cranial nerve damage symptomsof carotid resistance than the other two groups;Meningeal irritation occur, the youthgroup of patients with the other two groups were significantly different. The presentstudy, the first inspection of the head CT positive rate of94.32%,4.32%of the casespositive results are shown in3days after the review. In this study, elderly patients havehigher mortality than other groups.Rebleeding, cerebral vasospasm condition grading,seizures,Hunt-Hess grading,Fisher grading five factors negative correlation withclinical outcome satisfaction.Conclusion:1.More common in female patients, older patients≥60years of agegroup is more prominent.2.The major cause of the bleeding aneurysm rupture, followed byhypertensive arteriosclerosis;patients with different clinical manifestations in each agegroup.3.Head CT is the preferred method of examination, the DSA check thesearch for a cause of great significance.4.Rebleeding,cerebral vasospasm,seizures,Hunt-Hess grading, grading Fisher five factors negative correlation with clinical outcome satisfaction.5.≥60-year-old patients with a higher mortality rate.... | | Keywords/Search Tags: | Subarachnoid hemorrhage, Different ages, Spontaneous, Prognostic factors, GOS score | PDF Full Text Request | Related items |
