Pancreatic cancer is one of the most malignant,least resectable and most transferrabletumors. In recent years,more and more pancreatic cancers have been spotted.The numberof new cases and deaths is almost equal. It has been ranked as No.4in all deaths caused bycancer.More than80%of patients diagnosed are in the late period of the cancer and cannotbe treated with surgery[1].For those who can receive surgery treatment,the effect is notideal and it is easy for the cancer to relapse.Also,metastasis in vitro radiotherapy andsystemic venous chemotherapy effect is poor.Current treatment of pancreatic cancer inmiddle-late period is ineffective.Along with the development of image guide positioningtechnology and computer positioning treatment planning system (treatment plan systemTPS),radioactive125I implantation technology has been rapidly developed in cancertreatment. Especially,the effect in the treatment of pancreatic cancer in middle-late periodis satisfying[2].The CT guided radioactive125I implantation treatment for pancreatic canceris accurate in positioning, less harm, less side effects and quick in treatment.Close internalexposure can prevent the surrounding tissues and organs of pancreas from high doses ofradiation damage.This subject adopts this method to treat middle-late pancreatic cancer.Ithas achieved satisfying effects in the control of tumor growth,pain relief and patientsurvival rate improvement.ObjectiveTo study the clinical curative effect of radioactive125I implanted middle-late treatmentof pancreatic cancer guided by CTMethodsThis study is conducted by retrospectively analyzing the53cases of pancreatic cancerpatients (48males and5females, aging from47to70) in hospital from July2006to July2012.Among all the patients analyzed,there are39pancreatic head carcinoma cases,11pancreatic body carcinoma cases and3pancreatic tail carcinoma cases.All the patients have performed the preoperative CT,MRI and ultrasound diagnosis, and have been takenpuncture biopsy in CT environment,which have been confirmed of pancreaticadenocarcinoma.All the patients are divided into two groups of radioactive125Iimplantation treatment group and local chemotherapy infusion treatment group accordingto the randomized comparison principle.The cancer treatment effect,quality of life,painrelief and survival rate improvement are compared after the two groups received differenttherapies.ResultsTwo groups of patients in the follow-up for120months,review CT as a result twomonths after the operation, the total effective rate is45.3%,radioactive125I particle groupeffectiveness is53.8%,while local infusion group effectiveness is37.1%, and the treatmentparameters between the groups are significant,P=0.009,radioactive125I particle grouptherapy effect is better than that of local chemotherapy infusion group.Analyzing thechanges of life quality of the two groups,changes including signs and symptoms have takenplace in different degree before and after the treatment,but the radioactive125I particlegroup therapy effect is more significant.No serious complications (pancreatic fistula,alimentary tract hemorrhage, acute pancreatitis,enteritis,etc.) occur during the operation ofthe two groups,no death happens,class II,III pain relief obviously.The pain of theradioactive125I particle group begin to ease after mean34days,and the effective rate is77%,while the pain of local chemotherapy infusion group begin to ease after mean2025days,and the effective rate is44%.The postoperative pain relief of125I particle group isobviously higher than that of the local chemotherapy infusion group (P <0.05).The mediansurvival time of125I particle group is11.00months,6months cumulative survival rate is78%,12months cumulative survival rate is42%,while the median survival time of localchemotherapy perfusion group is9.75months,6months cumulative survival rate is70%,12months cumulative survival rate is30%.ConclusionThe treatment of radioactive125I implanted middle-late pancreatic cancer guided byCT is more superior than that of local chemotherapy drugs infusion. Efficiency is increasedand the clinical symptoms are improved;Recent analgesic effect is also obviously,especially in moderate to severe pain relief,working fast,and last long.Besides aboveadvantages,this method is operated simply,and cause less complications,etc.so it is a safeand effective method of middle-late minimally invasive treatment of pancreatic cancer.
