ObjectiveInvestigating the characteristics of suspicious breast masses in UE, to find the factorsinfluencing the quality of ultrasound elastography and evaluate the clinical application valueof ultrasound elastography in the diagnosis of suspicious breast masses.Materials and Methods1The choice of subjectsCollect the patients ever take examination in the Shandong province hospital from2012.1-2012.10,with total187breast masses(148benign,39malignant) in145femail patients.The age from22-78years old,mean year45.2±9.3.2Equipment and method:HITACH HI VISION PREIRUS with UE,4~11HZ, take examination of the breastordinary,and the date of Lesion size, distance from the nipple,lesion depth, and breastthickness at US should be record detailly. After that,by the UE technology,set the interestareal reasonably from the fat to the breast musle,so the ROI Should be set twice of the masstake proper pression way, should be evaluated when the pressure and compression frequencyindicating bar was displayed as2to3during examination of UE, get the UE image, we canalso observe the two-dimension and the UE image real time.3Statistical analysis:By spss ststistics software, estimate the factors of the size of the mass,the depth,thedistance to the nipple,and the location of the mass, then analysis of their association withimage quality by using the univariate analysis. Assessed the quality of elasticity images asinadequate, low, or high without histologic information. Sensitivities and specificities for thedifferentiation of benign from malignant masses on the basis of elastography were calculatedand compared between groups of quality scores by using the X2test method. ResultThe UE image quality result is that12masses get0point,73get1point and157get2point. According to univariate analysis, lesion size (P=.003), lesion depth (P=.000), lessbreast thickness where the lesion was located (P=.000) were significantly associated withhigher image quality. There was no correlation of image quality with distance from the nipple(P=.332). Multiple linear regression analysis showed that breast thickness at the location oftarget lesions was the most important factor influencing elasticity image quality (P=.000).Of the175masses with integrated image. according to the classify methode,≥4diagnosisedas maligent mass,≤3diagnosised as benign mass,total140benign mass and35maligentmass. There were significant differences in sensitivity between higher-quality andlower-quality images (87%VS55%, P<0.05) in the differentiation of benign from malignantmasses.ConclusionBreast thickness at the location of the lesion was the most important factor influencingimage quality at US elastography. Sensitivity for classification of benign and malignantmasses improved with higher quality scores.
