Objective:To explore the surgical treatment of pituitary adenoma.Methods: Retrospective analysis of215cases of pituitary adenomapatients from the first affiliated hospital of Chongqing medical university.All cases are clear as pituitary adenoma and have complete clinical date.Statistics content include physical date, clinical feature, incretion,iconography (MRI, CT, CTA), pathological examination, operativeapproach, rate of Totally removed, complications, adjuvant therapy,follow up with incretion iconography.Results: clinical feature: soft texture in female occupy86.3%(107/124),soft texture in male occupy68.1%(62/91), p<0.05.tough texture in femaleoccupy3.2%(4/124), tough texture in male occupy15.4%(14/91), p<0.05.soft texture in functional adenoma occupy84.3%(113/134), softtexture in non-function adenoma occupy69.1%(56/81),p<0.0167. toughtexture in functional adenoma occupy5.2%(7/134), tough texture innon-functional adenoma13.6%(11/81),p<0.0167. invasive grows inMicroadenomas occupy4.7%(2/43), invasive grows in macroadenomasoccupy18.9%(28/148), invasive grows in giant adenomas occupy 54.2%(13/24), the different occupy which with the three different sizeadenoma in invasive grows compared with each other is statisticallysignificant. Apoplexia in invasive adenoma occupy20.9%(9/43),apolexiain non-invasive adenoma occupy9.3%(16/172),p<0.05. apoplexia innon-functional adenoma occupy18.1%(19/81), apoplexia in functionaladenoma occupy5.5%(6/134),p<0.05. operation results: Totally removedadenoma by transsphenoidal occupy82.0%(150/183), Totally removed bytranscranial occupy71.9%(23/32), p>0.05. Microadenomas totallyremoved occupy97.7%42/43), Macroadenomas totally removed occupy78.3%(116/148),Giant adenomas totally removed occupy62.5%(15/24),microadenomas totally removed was significantly higher thanmacroadenomas and giant adenomas (p<0.0167).Soft adenomas totallyremoved occupy88.2%(149/169), Soft and fibrous mixed adenoma to tallyremoved occupy67.9%(19/28), fibrous adenoma totally removed occupy27.8%(5/18), there was significant difference between any two amongtotally removed adenomas with three different textures (p<0.0167).Invasive pituitary adenomas were totally removed occupy55.8%(24/43), Non-invasive pituitary adenomas were totally removed occupy86.6%(149/172)p<0.05.Conclusion: compression symptoms is the primary clinical manifestationsof pituitary adenoma tumor in this group patients. Female and functionaladenoma have more soft texture, male and non-functional adenoma have more tough texture. the longer diameter, the more invasive pituitaryadenomas. Non-functional adenoma and invasive adenoma have moreapoplexia. The choice of approach is the key to the treatment of pituitaryadenomas. Factors such as tumor size, texture,and growth pattern couldinfluence the treatment of surgery.
