Objective To explore the indications and curative effect ofPemberton acetabuloplasty for the treatment of developmental dysplasia ofthe hip(DDH) in preschool children.Methods A retrospective analysis of67cases(88hips) of DDHtreated with Pemberton acetabuloplasty or Salter pelvic osteotomy in2008-2011years was carried out, according to McKay clinical criteria forhip joint and the rectification of acetabular index(AI).Of the52patients(68hips) treated with Pemberton acetabuloplasty,the preoperative AI of53hipsare less than45°and that of15hips are greater than or equal to45°.15hips are acetabular dysplasia or subdislocation and53hips are dislocationof hip. The preoperative AI of all patients(20hips) treated with Salterpelvic osteotomy are less than45°.8hips are acetabular dysplasia orsubdislocation and12hips are dislocation of hip.Results The67cases were followed up for1-3years, averaged2years. Pemberton acetabuloplasty and Salter pelvic osteotomy both achieveexcellent result treating DDH with preoperative AI less than45°. Therectification of AI by them are almost equal to each other. Pembertonacetabuloplasty achieves excellent result both treating DDH with preoperative AI less than45°and treating the ones with preoperative AIgreater than or equal to45°. The rectification of AI of hips withpreoperative AI greater than or equal to45°is obviously larger than that ofones treated with Salter pelvic osteotomy. Pemberton acetabuloplasty andSalter pelvic osteotomy both achieves excellent result treating acetabulardysplasia,subdislocation of hip and dislocation of hip.Conclusion The clinical outcome of DDH treated with Pembertonacetabuloplasty in preschool children is excellent. The Pembertonacetabuloplasty makes less trauma and less influence on the shape of thepelvis and volume.It is one of the most ideal treatment modalities for DDHin preschool children.
