Influence Of Different Sensitive Constitution On Acupuncture Analgesia Effect And Its Cns Modulating Mechanism: A Resting-state FMRI Study

Abstract/Summary:
ObjectiveTo determine the analgesia effect in different sensitive constitution andinvestigate its underlying central nerve modulating mechanism.MethodsForty-five healthy volunteers were recruited and divided into threetypical groups according to Traditional Chinese Medicine theory onconstitution.that is insensitive group, normal group and sensitive group,with15cases in each group. Pressure pain threshold(PPT) was measured inevery group before and after acupuncture at Zusanli point(ST36).Thenpaired t test was used to compare the effect of analgesia among the3group.Two weeks later, resting-state BOLD fMRI scanning was carried outby using a gradient-echo echo-planar imaging sequence before and afteracupuncture at ST36. All the fMRI data were analyzed by the method ofamplitude of low frequency fluctuation(ALFF). Paired t-test was used toanalyze the data acquired before and after acupuncture. With the change of PPT to analysis the the analgesia effect in different sensitive constitutionand the response characteristics of central nerve in the process ofAcupuncture.Results1.Compared the PPT before and after acupuncture, normal group andsensitive group had obvious statistical significance except insensitivegroup.2. Compared with the image before acupuncture, the insensitive groupshowed significant decreased ALFF values in the left fusiform gyrus andleft inferior temporal gyrus. No increased ALFF was found in thisgroup.The normal group had significantly increased ALFF values in thebilateral cerebellum posterior lobe, brainstem, limbic lobe, left inferiortemporal gyrus, right caudate, and significantly decreased ALFF values inthe left postcentralgyrus, left precentral gyrus, and bilateral occipital lobes.The ALFF values in the sensitive group were increased mainly in thebilateral cerebellum posterior lobe,fusiform gyrus,inferior temporal gyrus,basal ganglia,frontal lobe,anterior cingutate,right supramarginal gyrus,rightsupplementary motor area,left insular lobe, while the regions withdecreased ALFF values were the left middle temporal gyrus,left postcentralgyrus, posterior cingutate,and bilateral occipital lobes.Conclusion1.The factor of persons with different acupuncture-related sensitive constitution influence the effect of acupuncture analgesia.2.There were widespread differences in ALFF values among theinsensitive group, normal group and sensitive group,such as the increasedand decreased ALFF and the change of response range.The normal groupcompared with the insensitive group, The changes of brain functional areaswere main in the brainstem, limbic lobe, caudate nucleus,cerebellumposterior lobe and occipital lobe. The sensitive group compared withnormal group, The changes of brain functional areas were main in basalganglia, prefrontal lobe, cingulate gyrus and supramarginal gyrus.differentresponds of various encephalic region network related may be the centralmodulating mechanisms on the reason of constitution influencing theacupuncture effect.
