| Objective: To investigate and analysis the short-term effectiveness of posterialpercutaneous interlaminar full-endoscopic technique for discectomy in the treatment oflumbar disc herniation.Methods: Between Aug.2012and Jun.2013,we treated36patients who diagnosed aslumbar disc outstanding disorder cases. Using Full-endoscopic Technique through theposterior interlaminar cut off nucleus pulposus,according to the incision, operationbleeding, operation time, the incidence of complications and postoperative on bed time,length of hospital stay, postoperative CT review for statistical analysis. After1day and1ã€3ã€6months follow-up, patients with low back pain and leg pain,VAS score evaluation wasadopted to alleviate the situation, ODI score evaluation of patients daily work and lifesituation, modified Macnab criteria to evaluate the clinical effect.Results: This group of34patients successfully completed in the endoscopic surgery. skinincision is6-7mm, intraoperative blood loss was about5-10ml, operation time was60-110minutes, the average time was78minutes.postoperative on bed time was8-24hours, on average was16hours.Postoperative hospital stay of48-96hours, on averagewas64hours. The CT scan displayed of nucleus pulposus has been completely removed.Postoperative1case with “burning kind of hyperalgesia†over the lowerextremities,symptomatic improvement through by relative treated; Previously did notappear, nerve root injury and vascular injury, bowel complications such as injury, infectionof intervertebral disc.1case for implant work casing teared dural turn to open surgeryrepaired,1patient because of nucleus pulposus out shift into the lateral crypt far and difficultly removed rerouted to open surgery;34patients were followed up for6month ormore (including12cases were followed up for12months,11cases were followed up for9months,11cases were followed up for6months). Preoperative VAS score7.85+7.85, theODI score50.14+6.56, postoperative in June VAS score1.0+0.05, ODI score7.8+4.28.the comparison between preoperative and postoperative has statistically significant (P <0.05). After6months according to the Macnab criteria evaluation of curative effect,25cases, good in5cases,4cases, was88%(30/34).Conclusion: posterial percutaneous interlaminar fixed-point puncture full-endoscopictechnique for discectomy in the treated with nucleus pulposus of lumbar disc herniationhas the advantages of small trauma,less bleeding,fewer complications,rapid recovery andshort term curative effect is reliable. |
