Objective:To investigate the dynamic change of levels of serum sufurctant protein D(SP-D) in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) before and after therapy. To study the correlation of SP-D with Procalcitonin (PCT)and C-reactive protein (CRP)and to explore the relationship between SP-D and lung function in AECOPD before therapy. Methods:Forty patients with AECOPD and thirty healthy people were enrolled. Venous samples were obtained to detect levels of SP-D,PCT and CRP respectively by enzyme linked immunosorbent assay (ELISA) method. The lung functions (FEVi,FEVi/FVC)were measured by Spirometer made in Japan.Results:1. The levels of serum SP-D in patients with AECOPD before therapy were (23.52±4.2)ng/L,significantly higher than those of healthy individuals(19.71±2.86)ng/L (P=0.000,<0.05). After two weeks’therapy the SP-D levels in patients with AECOPD decreased to(21.40±3.78)ng/L (P=0.037,<0.05),but still higher than those of the healthy individuals (19.71±2.86)ng/L(P=0.04,<0.05).2. The levels of serum PCT in patients with AECOPD before therapy were (664.13±184.84)ng/L, significantly higher than those of healthy individuals (553.37±141.38)ng/L (P=0.01,<0.05).3. The levels of serum CRP in patients with AECOPD before therapy were (63.27±68.58)mg/L, significantly higher than those of healthy individuals (10.4±3.8)mg/L (P=0.000,<0.05).4.The levels of serum SP-D in patients with AECOPD before therapy was positively correlated with PCT(r=0.418,P=0.007),but had no correlation with CRP.5.The levels of serum SP-D in patients with AECOPD before therapy was negatively correlated with FEV1(r =-0.478,P=0.026) and FEVi/FVC(r=-0.488,P=0.001). The levels of serum SP-D in GOLD4were (25.11±3.89)ng/L higher than GOLD3(20.96±3.78) ng/L(P=0.042,<0.05).Conclusions:1.According to the dynamic change of levels of serum SP-D in patients with AECOPD before and after therapy, indicating SP-D may be a marker which can reflect COPD with acute exacerbation.2.The serum level of SP-D in patients with AECOPD before therapy was positively correlated with PCT but had no correlation with CRP.3The serum level of SP-D in patients with AECOPD before therapy was negatively correlated with FEVi and FEV1/FVC.
