Economic Evaluation Of Infant Hepatitis B Vaccination In Ningbo City

Objective:1Investigate the economic burden of hepatitis B-related diseases patients inNingBo.2Evaluate the economic benefit of the present infant hepatitis B vaccine (HepB)immunization program of NingBo.Meathod: Stratified random sampling was used for hepatitis B-related diseases patients inNingBo to investigate the economic burden including direct and indirect costs caused by hepatitisB-related diseases in last year.Multiple linear regression models were conducted to determine thefactors linked to the economic burden. Randomly selected6hospitals with obstetric in NingBo,and inspect the cases history to compute mother hepatitis B surface antigent (HBsAg) screeningrate, mother HBsAg positive rate, hepatitis B vaccine (HepB) and Hepatitis B immuneglobulin(HBIG)vaccination rate of infant born HBsAg positive mother.Make use of Meta analysisto obtain Antibody to hepatitis B surface antigen(Anti-HBs) positive rate.Collect data about infantHepB vaccination rate and any other indexes from NingBo municipal center for disease contral andprevention(CDC).Finally construct multistage trees and Markov model to compute cost, benefitand Benefit-to-cost ratio(BCR).Result:664patients were included,finally.On average, the annual per hepatitis B-relateddiseases patients was74259.30Yuan in NingBo.The costs per patient for hepatocellular carcinoma,severe hepatitis B, cirrhosis, chronic hepatitis B and acute hepatitis B were142194.99Yuan,107797.25Yuan,99295.04Yuan,26838.65Yuan and18867.54Yuan, respectively. Occupation,religious belief, personal annual income, the total discharge diagnosis classification were thecommon factors for different costs.Totally,3631cases history were examined,of which287HBsAg positive,and actually copy276.The HBsAg screening rate, mother HBsAg positive rateand vaccination rate was100%,7.94%and91.30%, respectively. The cost of infant HepB fullyvaccination was28.29Yuan in NingBo. The net present values present infant HepB immunizationprogram of NingBo was31251.915RMB Yuan and BCR was16.48.Conclusion:1. The annual cost of per patient in Ningbo with hepatitis B-related diseases was74259.30RMB Yuan,1.98times that of personal income and1.13times of family income.The latter radio was much larger than the samiliar research ofShenzhen2. The proporation of hospital costs in total costs was more than60%. Primary liver cancer,cirrhosis, chronic hepatitis B patients in costs of purchase medicine was2times morethan per year outpatient costs. Occupation, religious belief, personal annual income, totaldischarge diagnosis classification were the main factors for costs.3. At present, the net present values of infant HepB immunization program of NingBo was31251.915RMB Yuan, the BCR war16.48.The economic benefit was good.4. Vaccination rates, Anti-HBs positive rate, were the sensitivity indexes influencing HepBvaccined program.
