Objective To investigate the mechanism of gastric bypass surgery (gastric bypass) forthe treatment of type2diabetic (non-insulin dependent diabetes mellitus,NIDDM) rats.Methods Sixty adult male SD rats with type2diabetes were randomly divided into twogroups:type2diabetes-operation group (A group) with30and type2diabetes-control group(Sham-A group) with30. Before and after surgery at fasting and post oral glucose30mincondition, the weight, glucose levels and insulin levels of the rats in each group were observeand recorded. Glucagon-like peptide-1and glucose-dependent insulinotropic polypeptide inplasma were detected by ELISA. The mRNA expression of glucagon-like peptide-1receptorand glucose-dependent insulinotropic hormone receptor in small intestine, pancreatic tissuewere detected by RT-PCR. Results The fasting blood glucose of A group rats decreased to5.85±0.72mmol/L from20.05±3.05mmol/L postoperative8weeks (P<0.05) and keep thelow level for weeks; the fasting blood insulin of A group rats increased to14.85±2.52mmol/Lfrom7.55±3.15mmol/L postoperative8weeks (P<0.05); the IAI of A group rats decreased to0.12±0.06from0.32±0.12postoperative8weeks (P<0.05); the fasting GLP-1of A groupraised to25.58±2.15pmol/L from6.86±0.85pmol/L postoperative8weeks (P<0.05); thefasting GIP of A group raised to20.28±2.08pmol/L from5.65±2.08pmol/L postoperative8weeks (P<0.05); much more significant at the post oral glucose30min condition; the GLP-lRmRNA expression in the pancreas of A group raised to5.38±0.65from1.01±0.02postoperative8weeks (P<0.05); the GIP mRNA expression in the pancreas of A group hasnot different with postoperative8weeks (P<0.05). Conclusion The GBP operation has asignificant treatment effect on type2diabetic rats,it plays an important role in the highsecretion of GLP-1;decreasing secretion of GIP;promoting the expression of glucagon-likepeptide-1receptor, and it promotes insulin secretion and increaes insulin sensitivity which iscaused by food into the lower segment of jejunum directly post-operatively.
