Objectives:To examine the possible predisposing factors for intraocular lens (IOL) dislocation and to study surgical outcomes of repositioning or exchange of dislocated IOLs.Methods:Medical records of68consecutive patients with sixty-eight eyes who underwent IOL surgery for dislocation between March2009to March2013were reviewed.Results:Age of dislocation is48.9±18.9years, male49eyes (72.1%), female19eyes (27.9%), left eye37(64.4%), right eye31(45.6%). The interval between IOL implantation and surgery was21eyes≤1year,8eyes1<y≤5years,26eyes5<y<≤10years,7eyes10<y≤15years, none15<y≤20years,1eye>20years. Dislocation sites were superior6eyes (8.8%), inferior11eyes (16.2%,3eyes dislocate within the bag), anterior chamber24eyes (35.3%), posterior chamber2eyes (2.9%), vitreous cavity20eyes (29.4%), pseudophacodonesis5eyes (7.4%). Possible major predisposing factors for IOL dislocation were posterior chamber damage by surgery(17 [25.0%]), trauma (7[10.3%]), capsule shrinkage (6[8.8%]), secondary implantation (2[2.9%]), anterior megalophthalmos (2[2.9%]), Marfan syndrome (1[1.5%]), after NdrYAG surgery (1[1.5%]), and no identifiable factors (32[47.1%]). Mean uncorrected visual acuity (VA) improved significantly after IOL surgery (P=0.0001). There was no significant difference of uncorrected VA improve between repositioning and exchange surgery (P=0.212).Conclusions:There is no significant relation with IOL dislocation and age, incidence male is higher than female, interval between IOL implantation and surgery was≤1year and5<y≤10years, Dislocation sites were anterior chamber and vitreous cavity, possible major predisposing factors for IOL dislocation were posterior chamber damage by surgery and trauma, after IOL surgery, uncorrected VA improved significantly, there was no significant difference of uncorrected VA improve between repositioning and exchange surgery.
