Objective:To explore the clinical value of procalcitonin in the diagnosis of severepneumonia.Methods:In the Sixth Affiliated Hospital of Xinjiang Medical Universityhospital,Retrospective analysis was made of95cases pneumonia patients from September2011to September2012.Normal control group (50cases),the selected cases were dividedinto ordinary pneumonia group (50cases),severe pneumonia group (45cases).Normalcontrol group:Laboratory tests PCT,IL-6,CRP and WBC was serum in physicalexamination.Case group was the determination of serum the PCT,IL-6,CRP,WBC levelbefore hospital treatment,48h,72h,prior to discharge or death,and did the PSI score,APACHEⅡscore.Results:Serum PCT is not elevated in the normal control group,whereasincreased slightly in ordinary pneumonia group and significantly in the group of severepeumonia;IL-6,CRP and WBC is not elevated in the normal control group,whereasincreased significantly in the ordinary pneumonia group and severe pneumonia group.Serum PCT level in severe pneumonia group and ordinary pneumonia group differencewas statistically significant (P<0.05).IL-6,CRP,WBC level in severe pneumonia groupand ordinary pneumonia group difference was not statistically significant (P>0.05);PCT,IL-6,CRP,WBC levels in ordinary pneumonia was significantly higher than in normalcontrol,the difference is significant;continuous monitoring of severe pneumonia in serumPCT,IL-6,CRP,WBC levels found before in antibiotic use the serum PCT quickly rise,when the treatment is effective,the decline in serum PCT fast,and IL-6,CRP,WBC level ofinfection control remains a high level,to slow its rate of decline in serum the PCT;SerumPCT:there were significant differences in Severe bacterial pneumonia and the nonbacterialsevere pneumonia group.serum PCT levels with PSI score,APACHE II score correlationcoefficients were0.686,0.702,its relevance are higher than IL-6,CRP and WBC. Conclusion:PCT can be used for monitoring changes in condition and assess theprognosis of patients with severe pneumonia,Its effect is better than IL-6,CRP and WBC.
