ObjectiveTo observe the differences of serum leptin concentration in patients withnon-coronary heart disease, stable angina, unstable angina and acute myocardialinfarction, to observe the correlation among serum leptin level, risk factors of coronaryheart disease, biochemical markers and Gensini score, to explore the role of serum leptinlevels for risk stratification in coronary heart disease.MethodsAccording to coronary angiography,120patients were enrolled in the present study,which are divided into4groups: the non coronary heart disease group, stable angina(SAP), unstable angina pectoris (UAP) and myocardial infarction group (AMI),respectively.The angiographic characteristic of coronary artery atherosclerosis wasdefined with the method of modified Gensini score. Clinical materials of all patients werecollected. The serum leptin, hs-TnT, NT-proBNP, hs-CRPconcentrations of the4groupswere determined by ELISA. Gensini score and vascular lesions were detected by thecoronary angiography. The correlation among serum leptin level, risk factors, hs-TnT,NT-proBNP, hs-CRP of coronary heart disease (CHD) were analyzed, the relationshipbetween the levels of serum leptin and the Gensini score were investigated respectively.Results1.Concentration of serum Leptin in AMI group was significant higher than the controlgroup and SAP group(P <0.001). It is no significant difference between in the controlgroup and SAP group (P>0.05).2.Serum Leptin was dramatically increased in multivessel disease group thandouble-vessel disease group and single-vessel disease group (P <0.001).3.The concentration of Leptin had increasing tendency follow up the scores ofGensini score, and compared with mild vascular lesions group, and it was provided with significant difference both of Moderate vascular lesions group and severe vascularlesions group (P <0.001).4.Concentration of serum Leptin was elevated by the rising of Smoking index, thedifference was statistically significant (P <0.001).5.Serum NT–ProBNP concentration in AMI group was obviously elevated than UAPgroup, SAP group and the control group, the difference was statistically significant (P <0.001).Concentration of serum NT-ProBNP in multivessel disease group was extremelyelevated than double-vessel disease group (P=0.005) and single-vessel group (P <0.001).6.Serum high-sensitivity troponin T in AMI group was visible higher than UAPgroup,SAP group and the control group, the level of hs-TnT in UAP group was higherthan SAP group and the control group, the difference was statistically significant (P <0.001).high-sensitivity C-reactive protein in AMI group was visible higher than UAPgroup, SAP group and the control group, the level of hs-CRP in UAP group was higherthan SAP group and the control group, the difference was statistically significant (P <0.001).7.Compared with the control group, concentration of serum Creatinine(Scr)in AMIgroup was extremely raised (P <0.001), the UAP group and the SAP group were higher(P <0.01). Platelet count (PLT) in AMI group were elevated than that of the controlgroup (P <0.001) and the UAP group (P<0.05). PLT were rising along with theincreasing of Gensini score, group C was higher than group A (P <0.001); group C washigher than group B (P=0.003).8.Bivariate correlation analysis showed that the concentration of Serum Leptin hadpositived relevance with hs-TnT,NT-proBNP,hs-CRP,Gensini score(P <0.001)CK-MB(P=0.035), and smoking index (P=0.033). Conclusion1. The results indicated that concentration of serum Leptin had increasing tendencyfollow up the degree of coronary artery lesions. Therefore, it can be used as a valuablemarker to evaluate acute coronary events.2. Concentration of serum Leptin, hs-TnT and NT-ProBNP, hs-CRP,PLT, Scrincreased with the severity of coronary artery lesions, and it reflect the severity ofmyocardial ischemia and myocardial injury.3. Concentration of serum Leptin,combination of other commonly biochemicalmarkers can improve the accuracy of the risk stratification of patients with CHD.
