Objective Through comparing the effect of medical insurance mixed payment methodon medicare patients with which based on the traditional payment method offee-for-service in the departments of orthopedics and general surgery in a tertiaryhospital and analyzing the variance of indicators such as medical costs,medicalquality,service efficiency,patient satisfaction and so on,to find the problems that needfixing and provide the basis for improving medical quality, reducing medical costs andpromoting the reasonable allocation of health resources.Materials and methods The study selected500cases of medicare patients inorthopedics which were carried out by the mixed payment method as experimentalgroup and584cases which were implemented by FFS as control group.At the sametime,it set1281cases as experimental group and1123cases as control group in generalsurgery.All the information was collected by means of retrospective analysis fromhospital information system in a tertiary hospital between July1st,2011and June30th,2012. The implementation effect of different payment methods was evaluated byanalyzing the indicators such as work efficiency, economic efficiency and theperformance appraisal.Results There was not statistical difference in terms of gender, age and admissionbetween two groups in departments of orthopedics and general surgery(P>0.05).Thetotal hospitalization expenses,drug cost,material cost and bed cost in experimental group were lower than those in control group, and there was statistical difference betweenthem (P<0.05).While the treatment cost,care services as well as the cure rate varied littleand showed no statistical difference(P>0.05).The patient satisfaction in experimentalgroup was higher than that in control group,and there was statistical difference betweenthem(P<0.05).The examination fees in orthopedics experimental group were lower thanthat in control group,and there was statistical difference between them(P<0.05).Whilethe examination fees in general surgery changed little between two groups and showedno statistical difference(P>0.05).Conclusion As an important part of the new health care reform,the improvement ofmedical insurance payment method is the most direct and sensitive part related toeconomic interests of all parties during the process of medical insurance reform.It has apivotal role in monitoring medical services and curbing excessive medicaltreatment.The implementation of medical insurance mixed payment method in internalhospital can guide medical personnel standardizing clinic behavior,control medicalexpenses,ease the burden on patients,reduce unnecessary health resources consumptionand make the hospitalization fee structure tend to be reasonable on the premise ofguaranteeing the quality of medical care.
