ObjeetiveTo observe the effect of anti-glaucoma medicines and surgery on ocular surface and SP in glaucoma tears.Method1.Study group:Thirty patienis (40eyes) diagnosed as glaucoma combining with cataract were enrolled in this group, whose age was ranged from40to70. Case inclusion criteria:patients undergoing anti-glaucoma medications. Case exclusion criteria:(1) patients suffering from diabetes and other systemic diseases;(2) people with local diseases affecting the ocular surface (such as blepharitis, dacryocystitis, keratoconjunctivitis sicca syndrome, pemphigoid, etc.);(3) people who had history of eye surgery;(4) people who had history of contact lens wear. Pre-operative examination of the patients about the ocular surface and substance P after application of the anti-glaucoma medications were considered to be observed object. Examination of the patients after anti-glaucoma medicines and surgery were considered to be the object to observe change after using medicines alone and medicines and surgery. We observed the score of preoperative ocular surface changing, clinical trials value of Schirmer I and clinical trials value of BUT, and collected the basal tears to test the SP by ELISA. All the patients aceepted phacotrabeeulectomy conformed by the same surgeon. The change of ocular surface seore, clinical trials value of Schirmer I and clinical trials value of BUT were observed after45days,3months,6months. We also collected the basal tears to test the SP by ELISA at the same time.2.Control group:Thirty volunieers (30eyes) with no contact lens wearing history were enrolled in this group, whose age was ranged from40to70. Case inclusion criteria:no history of dry eye symptoms, eye disease, eye surgery, wearing contact lenses history, ocular allergy and diabetes; Schirmer I test>10mm; BUT>10s. We collected the basal tears to test the SP by ELISA.3. Comparison and analysis: (1) Difference of the number of dry eyes of the patients in the test group before and after surgery.(2) The SP of the basal tears between the control group and the test group before surgery.(3) The changing of the SP in the basal tears in the patients of study group before and after surgery.(4) The effect of kinds of anti-glaucoma medicines and using time on the ocular surface and SP, as well as SP on the tissue of ocular surface before surgery.(5) The effect on the SP in the basal tears of ocular surface after glaucoma combined with cataract surgery, and the main role in the healing process after glaucoma combined with cataract surgery.Result1. SP concentration in the basal tears of ocular surface from study group was258.16±8.81pg/mL, but the control group’s was336.37±6.80pg/mL, there was statistical difference between each other (P<0.01). On using time, the SP was254.05±8.20pg/mL and262.27±7.52pg/mL in the>3months group and≤3months group, the difference was statistically significant (P<0.05); On types of medications, the SP was264.78±5.50pg/mL,258.00±10.20pg/mL and254.16±6.85pg/mL in the medication alone group, two kinds groups and three kinds group, the difference was statistically significant (P<0.05), and the SP in the anti-glaucoma medications treatment group had the negative correlation with dry eye (r=-0.584, P<0.05).2. The SP in the basal tears was336.37±6.80pg/mL and408.93±16.95pg/mL in the control group and45days group, the difference was statistically significant (P<0.05); The SP in the basal tears was310.17is±13.41pg/mL in the3months after surgery, which has the statistically significant comparing with the control group (P<0.05); The SP in the basal tears in the6months group was268.68±11.91pg/mL, which was significantly lower than the control group (P<0.05).3. The SP in the basal tears was258.16±8.81pg/mL and408.93±16.95pg/mL in the preoperative group and45days group, the difference was statistically significant (P<0.05); The SP in the basal tears was310.17±13.41pg/mL in the3 months after surgery, which has the statistically significant comparing with the preoperative group (P<0.05); But the SP in the basal tears in the6months group has no significant difference with preoperative group.Conelusion1. Glaucoma medications can reduce the SP content in the basal tears; Increase treatment time and types of medication would reduce SP, accompanied the rise of dry eye. The SP was negatively correlated with the score of ocular surface changing, suggesting that SP can provide the nutrition and stable ocular surface.2. Glaucoma medications and surgery can change the SP in the basal tear of ocular surface. The latter can rise the SP to participate the repairing of tissue. But the dry eye increased, which advising that SP mainly participate the repairing of tissue, but the surgery would affect the tear film and ocular surface, leading to tear film instability, causing eye discomfort, dry eye occurs.3. To provide new ideas and theories on the treatment with glaucoma medications and surgery, which would cause eye irritation and dry eye; To learn the effect of SP on the glaucoma patients who had treatment with glaucoma medications and surgery.
