ObjectiveTo evaluate the exodeviation, binocular vision and ocular accommodation functions and so on pre and postoperation in intermittent exotropia patients who with basic and convergence insufficiency type, and to compare with the differences in eath type and between the two types pre and postoperation. To evaluate these differences whether there is any relationship with the post-eye position and its maintaining.MethodsCase-grouped study:Eighty-seven patients aged from8to30years old were selected from the IXT patients who visited Tianjin Eye Hospital from January2012to October2012.49patients in basic type was defined group1and39patiens in convergence insufficiency was defined group2. All patients had corrected visual acuity which not less than1.0and the horizontal deviation was not less than15PD and could be controlled sometimes. All patients were able to cooperate with all inspections. Meanwhile patients with surgical history, amblyopia, nystagmus, other ocular diseases, abnormal eye movement and other types of strabismus were exclued.Each patient had underwent refractive examination before admitted to hospital, and then complete examination was carried out on1day before surgery and1day,6weeks and6months after surgery, which included visual acuity, the distance and near deviation, fusion, distance and near stereoacuity, accommodation amplitude, accommodative response, accommodation facility, AC/A ratio, NPC and so on. To compare these pre and postoperation parameters between the two groups, and try to find out if there was any difference. Then to analyze the causes of the differences. A P value<0.05was considered statistically significant(SPSS for Windows, Version16.0).Results1. There were no differenes between the two groups at some aspects including male-female ratio, age, age of onset, deviant eye and family history, P>0.05; The separate spherical equivalent of the deviant eye was-0.87D(SD=0.38),-1.73D(SD=0.57) in the two groups; The separate astigmatism was0.00D (0.00~0.75),0.75D (0.13-1.75); The percentages of anisometropia(>1.0D) was18.75%,28.94%separately; and it all showed a difference in the three aspects, P<0.05.2. The patients’horizontal deviation in group1was mean27.50PD(32.50-53.88), while35.OOPD (30.00~45.00) in group2before surgery, and there were no differenes between them (P=0.219). The deviation in group1was mean3.OOPD (3.00~5.00), while7.00PD (5.00~10.00) in group2at6months after surgery, and it showed a statistical difference between them (P=0.000); and it showed a statistical difference in eath group pre and postoperation (P=0.000, P=0.000). The success rate of surgery in all patients was66.67%. and it was75.51%in group1while55.26%in group2, and it showed a statistical difference between them (χ2=3.948, P=0.047).3. The mean fusion of convergence was6.31°(SD=2.42) before surgery and20.08°(SD=5.70)6months after surgery in group1; It was4.64°(SD=1.73) before surgery and13.17°(SD=6.43)6months after surgery in group2. It showed a statistical difference pre and postoperation in eath group, P=0.000,and there was a statistical difference between the two groups before and after surgery, P=0.000. There was no statistical difference of distance and near stereoacuity pre and postoperation in eath group and between the two groups before surgery, P>0.05. There was a statistical difference between the two groups after surgery of distance stereoacuity, P=0.029; while it showed no statistical difference in near stereoacuity aspect, P=0.437.4. There was no difference of AA in eath group and between the two groups pre and postoperation, P>0.05. and it showed a statistical difference between the deviant eye’s mean AA and the contralateral eye’s pre and postoperation, P<0.05. The deviant eye’s mean AF separately was9.42cpm (SD=3.26),9.36cpm (SD=2.58) pre and postoperation in group1, and they were7.84cpm (SD=4.23) and7.97cpm (SD=4.04) in group2; There was no statistical difference in eath group pre and postoperation, P>0.05. and it showed a statistical difference between the two groups, P<0.05. The separate NPC pre and postoperation was6.69cm (SD=3.45),5.61cm (SD=1.76) in group1, and it was10.26cm (SD=5.21),8.17cm (SD=4.43) in group2. It showed a statistical difference in eath group and between the groups pre and postoperation, P<0.05. The separate AC/A ratio pre and postoperation was3.02△/D (SD=1.41),2.18A/D (SD=1.56) in group1, and it was1.33△/D (SD=0.74),1.25△/D (SD=0.48) in group2, there was no statistical difference in eath group pre and postoperation, P>0.05, while it showed a statistical difference between the two groups,.P=0.000.Conclusions1. There were more myopic, astigmatism and anisometropia patients with convergence insufficiency type IXT than those with basic type IXT.2. The success rate of the patients’ with convergence insufficiency type IXT was lower than those with basic type IXT.3. The fusion of the patients with IXT would be improved after surgery.4. The deviant eye’s AA and AF was worse than the contralateral eye in the two groups. The NPC of the groups postoperation were smaller than preoperation.
