The Prognostic Analysis Of Breast Conserving Therapy For Early Breast Cancer

Abstract/Summary:
Objective Analyzed the clinical and pathological features of breast conserving therapy with early breast cancer. Research the relevant factors of impacting local recurrence, metastasis and survival after breast-conserving therapy, to give some advices to our Chinese clinicians.Methods A total of720early breast cancer patients treated with breast conserving therapy in Tianjin Medical University Cancer Hospital between January1997and October2010were reviewed.Summing up the patient’s clinical and pathological features, Analysis the relationship between clinicopathological factors related to the relationship and the impact on patients with local recurrence, distant metastasis and overall survival time.Followed-up date up to July2007. Survival time related definitions:disease free survival, the time which beginning from the date of surgery to recurrence or metastasis of any part of the body; Overall survival,the time which beginning from the date of surgery to the date of patients dead.death including tumor-related death and non-tumor-related death. Statistical analysis using SPSS17.0software, Description of the clinical data based on percentage, Clinical pathology groups compared using Chi-square test, Kaplan-Meier survival rate was calculated, Log-rank test were used for survival significance test, Cox proportional hazard line multivariate analysis, The definition of P<0.05was considered statistically significant.Results Whole group of720patients were female, aged19-87years old, With a median age of46years old,(?)35years old,126cases,542cases of36to65years,>65years of age in52cases;508cases of premenopausal patients. Whole group of patients with a median follow-up of65months.There were16patients dead,are all tumor-related death,17patients had local recurrence firstly,among these cases6patients had lpsilateral breast recurrence,and3patients were dead finally,5-year local recurrence rate is1.1%.24patients had distant metastases firstly,13patients died,and the remaining patients were survival.5-year disease-free survival and overall survival rates were95.8%,98.9%.Univariate analysis results show that:Age, margin status, postoperative radiotherapy, endocrine therapy related with local recurrence (P<0.05); Age, surgical approach, lymph node metastasis numbers, histological grade, endocrine therapy for patients related with distant metastasis; Age, number of lymph node metastasis, histological grade, pathological TNM stage, postoperative radiotherapy treatment and endocrine therapy related with disease-free survival time (P<0.05); Pathological TNM stage, the number of lymph node metastasis, histological grade, surgical margin status, postoperative radiation affecting overall survival time (P<0.05). Multivariate analysis results showed that:Age is an independent factor with disease-free survival (P<0.05); Histological grade, postoperative radiotherapy affecting overall survival in patients as independent prognostic factors (P<0.05).Conclusions Early breast cancer with breast-conserving surgery is entirely feasible, they can choose breast-conserving surgery firstly. But must ensure that negative margins and radiotherapy treatment in order to reduce the local recurrence rate and effectively improve the survival rate of patients.and the youth is not breast-conserving surgery contraindication, the patients who had poor prognosis were related with the poor molecular markers.
