Incarcerated rectal prolapse patients who undergo a perineal rectosigmoidectomy have a high incidence of anastomosis leak. Here we report that a modified two-stage perineal rectosigmoidectomy was used to treat3extremely rare cases with acute incarcerated rectal prolapse. The outer layer of the intestinal canal was excised and the part of redundant bowel transected in the first stage. In the second stage, the inner layer of intestinal canal was anastomosed with the outer layer of anal canal when the rectosigmoid tissues achieved adhesion two weeks later. During the follow-up that ranged from18to24months (3,6,12,18, and24months), all the three patients had healthy physiological characteristics, without anastomosis leak, bleeding, wound infection and recurrences. We suggest that the modified rectosigmoidectomy may be a good choice for the unusual incarcerated rectal prolapse case.
