Objective:To explore the impact of pancreaticoduodenectomy (Panereatoduodeneetomy,PD) and Pancreaticojejunostomy on the incidence of postoperative complications, and choose a safe anastomosis.M-ethods: A retrospective study was conducted to130routine pancreatoduodenectomy cases, which were selected from January2006to December2011in the1st Affiliated Hospital of Xinjiang Medical University, among them,74cases were performed pancreaticojejunal end-to-side anastomosis,35cases were performed pancreaticojejunal end-to-end anastomosis,21cases were performed end-to-side “mucosa-to-mucosa” anastomosis. Results: Postoperative complications were occurred in44patients (44/130), occupied33.8%. Among them, pancreaticojejunal end-to-side anastomosis in26cases (26/74), the rate of complication was35.1%; pancreaticojejunal end-to-end anastomosis in11patients (11/35), the rate of complications was31.4%, end-to-side “mucosa-to-mucosa” anastomosis in7cases (7/21), the rate of complication was33.3%. By comparison, there are not statistically significant (P>0.05) in the incidence of postoperative complications of these3surgical methods, which indicates that there is no direct relationship between Panereatieojejunostomy and the incidence of postoperative complications. Conclusions: the same surgical method is not suitable for all patients, regarding to the operative techniques, clinical experience, and to try to minimize postoperative complications, to choose a safe anastomosis based on the patient’s specific circumstances.
