Abstract/Summary:
ObjectiveTo evaluate the value of treadmill exercise test(TET) and24-hour dynamic electrocardiogram in the diagnosis of coronary artery disease, which compared with the results of coronary angiography.Methods81cases suffered with similar coronary heart disease symptom were52.0±5.5year-old(38-72year-old), of which42cases is male,39cases is female. They were all checked by treadmill exercise test(TET),24-hour dynamic electrocardiogram (12leads Holter) and selective left, right coronary artery angiography (CAG), and then comparative analysis the value of TET and Holter according to the results of CAG.Results1. According to the results of CAG,20cases were CAG negative ones, while others(61cases) were suffered with coronary artery disease, there were28cases with single vessel disease:the left anterior descending artery(LAD) lesions (15cases),the left circumflex(LCX) lesions (6cases), and right coronary artery(RCA) lesion(7cases); there were26cases with double_vessel disease:LAD combined LCX lesion(9cases), LAD combined RCA lesions (10cases), LCX combined RCA lesion (7cases); there were7cases with three vessels:the left main coronary artery(LM) lesions (2cases),LAD,LCX and RCA both lesions (5cases).2. The sensitivity of TET was72.1%for the diagnosis of coronary heart disease, while the specificity was80%;3. The sensitivity of12leads Holter was63.9%for the diagnosis of coronary heart disease, while the specificity was75%.4. To detected the patients with coronary artery lesions, there was no statistically significant difference in the sensitivity of TET and12leads Holter for the diagnosis of coronary heart disease as well as in the specificity of them(P>0.05).5. To detected the patients with different range of coronary artery lesions,there was no significant difference between the value of TET and12Holter (P>0.05)6. TET and12leads Holter could detect the patients with the coronary multivessel disease or double branch lesions more effectively and efficiently than to detect the patients with single branch lesion. The difference was statistically significant (P<0.05)Conclusion:1. The sensitivity of TET and Holter are high in diagnosis of coronary artery disease as well as the specificity of them. Meanwhile, TET and Holter are the relatively convenient, safe and reliable noninvasive method in diagnosis of CHD nowadays, which would have a promising application prospect in future.2. TET and12leads Holter could detect the patients with the coronary multivessel disease or double branch lesions more effectively and efficiently than to detect the patients with single branch lesion.3. The combination of TET and Holter can improve the accuracy in diagnosis of coronary heart disease.4. TET and12leads Holter with relatively high false positive rate and false negative rate should be combined with other tests and examinations to diagnose CHD, which could reduce the false positive rate and false negative rate.
