[Background]Endometriosis occurs in10%-15%women of reproductive age, resulting indysmenorrheal, chronic pelvic pain, dyspareunia and infertility, which severely impairfemale health and living quality. The damage of fertility caused by endometriosis isextensive, with troublesome therapy and unclear pathogenesis. It was about30%-50%patients with infertility, of which caused by severe EMs is rather tough to treat. Since1978, the first baby, British Louise Brown, with in vitro fertilization treatment was born,assisted reproductive technology (ART) has further developed, providing new methodfor treatment of EMs. Recent Meta analysis reported that fertilization rate andimplantation rate decreased markedly in patients with EMs, which is suggested to due tothe poor endometrial receptivity and oocytes quality. Our study purposed to investigatewhether the poor oocyte itself caused by EMs negatively influenced IVF outcome.[Objective]To investigate the mechanism of decreased oocyte quality at cellular ultrastructureand molecular level in patients with EMs.[Subject and methods]1) All the cases from infertile patients who received IVF treatment in theReproductive Medicine Center of Anhui Provincial Hospital Affiliated to Anhui MedicalUniversity, in the period of February2011to December2011.98patients (age21-38year, infertility duration0.5-17years) with EMs diagnosed by laparoscopy orlaparotomy and without recurrent cyst, PCOS, uterine adenomyosis and fibroids were included as study group.150homochronous patients with infertility caused only by tubefactor were included as control group. Continuous datas were compared using t test andrates were compared using Chi square test.2)35oocytes abandoned in IVF cycles from patients with Ems and tube factorwere collected for ultrastructure observation under electron microscopy in the period ofApril2012to Janurary2013.3) Homochronous MI oocytes abandoned in ICSI cycles from patients with EMsand male factor were collected for mtDNA copies detection using Q-PCR method.[Results]1) No difference was observed in number of oocyte retrieved, perfect embryos rate,clinical pregnancy rate between EMs group and control group. But normal fertilizationrate and implantation rate decreased in EMs group.2) In EMs group, organelles, such as cytoplasmic CG,lysosome,golgiapparatus,rough endoplasmic reticulum and ribosome, optical density analysis ofmitochondria, mitochondrial content, the proportion of length to diameter, were notdiscovered abnormal verse those in control group. But mitochondrial density reducedand abnormal mitochondrial increased markedly in EMs group3)The mtDNA copies decreased obviously in EMs groups.[Conclusions]1) Normal fertilization rate and implantation rate decreased in EMs group.2) Mitochondrial density reduced and abnormal mitochondrial increasedmarkedlyin EMs group3) The mtDNA copies decreased obviously in EMs groups.
