ObjectiveTo observed the characteristics of myocardial strain and rotation in patients with cardiacinsufficiency by using speckle tracking imaging,in order to evaluate systolicdyssynchrony of left ventricular and its relationship with left ventricular systolicfunction.MethodsThis research used standard dynamic images of basal,papillary muscle and apical-LVshort-axis from thirty patients with cardiac insufficiency and thirty healthyvolunteers,meanwhile,apical long axis chamber,4chamber and2chamber views ofthree consecutive cardiac cycles were acuquired.Parameters included peak systolicstrain,peak rotation,the maximum difference and standard deviation of time to peaksystolic strain and peak rotation among18segments were measured and calculated with2D strain software respectively.At the same time, left ventriclar ejection fraction(LVEF)was calculated through real time tri-plane LV volume rule.Results1.Compared with healthy volunteers,the longitudinal,radial,circumferential peak strainand rotation were reduced in patients with cardiac insufficiency(P<0.05),the time topeak strain and peak rotation were inconsistent.2.Left ventricular systolic dyssynchrony indices(SDI)were prolonged significantly compared with healthy volunteers(P<0.01),and have good correlation with LVEF.Therewere a higher correlation between radial index and LVEF.Conclusion1.Speckle tracking imaging can evaluate the characteristics of muscle strain and rotationin patients with cardiac insufficiency from longitudinal,radial and circumferentialdirection accurately and synthetically.2.Speckle tracking imaging is a new kind of simple,noninvasive,repeatability andangle-independent method for analysis the motion of myocardium quantitatively.3.Myocardial strain,rotation curves and their waveform are consistent,time to peak areconcentrated relatively in healthy volunteers;myocardial strain,rotation of apical and leftventricular twist are reduced significantly in patients with cardiac insufficiency,thewaveform of these curves are disorder.Time to peak is inconsistent.4.SDI have good correlation with LVEF,can be used as the effective index for evaluatethe systolic dyssynchrony of left ventricular. There is a higher correlation betweenradial index and LVEF.
