【Objective】Speckle tracking technology(STI) is a new ultrasound technology, whichcan analyse not only the speed of myocardial movement, the strain, the strain rateand backscattering integra quantitatively, but also the twist of left ventricular. STIis not dependent on the angle, compared with Tissue Doppler Imaging and strainrate imaging. The mortality rate increases when diabetes have cardiaccomplications. Diabetic cardiomyopathy(DCM) is a diffuse cardiomyopathy,which is caused by diabetes. In DCM, myocardial cytology function may beabnormal. Diastolic dysfunction is a major characteristic in the early DCM. Andsystolic dysfunction is a later development after diastolic dysfunction occurringin DCM. The study aimed to evaluate the change of the systolic and diastolicfunction by strain and twist, obtained by using STI.【Methods】Forty-seven patients with type Ⅱ diabetes, divided into two groups based on a merger of the microvascular complications,28people without microvascularcomplications while19people with microvascular complications, and twenty-seven control peopleunderwent the examination of echocardiography. All of the original data wasimported into EchoPAC workstation, analyzing and quantifying the LV strain andtorsion. The experiment will be divided into two parts:part one:1、LVEDV and ESV will be measured in two dimension ultrasound.LVEF will be counted by the Penn-cube formula and adjusted Simposon.2、Tomeasure Sl、Sr and Sc by the result of STI analysis.part two:1、To measure E, A, E’, A’, IVRT et al in the picture of the mirtraland aortic value inflow and Mitral valve moving ring by TDI.2、To measurePtw、PTV、PUV、TPTV、TPUV、UntwR by the result of STI analysis. Then theparameters with significant difference were made correlation analysis【Results】Part one: compared with the control group, EDV, ESV, and EF of DM Ⅰ andDM Ⅱ group have no significant change, but the Sl both reduced, and the Scalso reduced in DM Ⅱ group.Part two: there were significant differences in E’/A’、E/E’、IVRT、Ptw、PUV、UntwR、TPTV and TPUV among DM Ⅰ、DM Ⅱ group and control group, PTVreduced in DM Ⅱ group compared with the control group, and PTV、PUV、UntwR、TPTV and TPUV are different significantly compared with DM Ⅰ group.UntwR and E/E’ have a certain negative correlation(r=-0.5, p<0.01), but UntwRalso have a positive correlation withE’/A’(r=0.37,p<0.01)。【Conclusions】1、 Sl reduced in the early stage of diabetes, and decreasedsignificantly With the emergence of DM microvascular complications. It shows that systolic function may change in the early stage of diabetes, and it will bemore obvious with the advent of microvascular complications.2、 the parameters of torsion, obtained by STI, may be used to evaluate thediastolic function of left ventricular, and UntwR may be the most significant one.
