[Objective] To compare the influence of gingival tissue; sulcus width and clinical effects of mechanical method(gingival retraction cords-nodrug),mechanical-chemical method(gingival retraction cords impregnated with15%aluminium chloride) and YAJING gingival retraction paste(made in china).Synthetic evaluation the effect and gingival irration of the three gingival retraction methods and to provide reference for YAJING paste of clinical application.[Methods]15volunteers studying and woking in affiliated stomatological hospital of Kunming medical university and36patients all suit for the inclusion criteria were selected.Thirty maxillary first premolar of fifteen volunteers divided into three groups randomly and treated with pure cord,cord with15%ALCL3and gingival retraction paste (YAJING) respectively on the buccal aspects.The quantity of gingival crevicular and the active of aspartate aminotransferace(AST) and alkaline phosphatase(ALP) in GCF were measured at baseline and1d,3d,7d,14d after application by electron analytical balance and automatic biochemistry analyzer.To assess the level of gingival inflammation and the outcome of gingivitis of the three methods.Impressions were made before and after gingival displacement by silicon rubbers,then casts were made.The models were scanned and measured by Denmark3shape dental model scanner and software.Gingival sulcus width were measured.36patients with63teeth divided into3groups randomly(21teeth in each group) and treated with the three methods,then to evaluate the clinical effect(tooth preparation; hemostatic effect, impression; model; margin) and the feeling of the patients of the three methods.[Results](1) Compared with baseline the quantity of GCF of the three methods presented an ascending tendency after retraction,two cords reached the top at1d(P<0.01),the paste at3d(P<0.01),and at1d the quantity of GCF of cord with15%ALCL3was significantly higher than others(P<0.01),then all the three groups began to fall,the pure cord gradually recovered close to original level at7d,the cord with15%ALCL3and the paste at14d.(2)The changes of the active of GCF-ALP of two cords basically the same as the GCF,the active of GCF-AST of two cords reached the top at1d,recovered close to original level at14d.The changes of the active of GCF-ALP and GCF-AST of paste(YAJING) basically the same as the GCF.(3)The measurements of the sulcus width before and after retraction shows that the sulcus width of cord with15%ALCL3was significantly greater than pure cord(P<0.01),and YAJING paste was greater than two cords(P<0.01).(4) Less patients treated with pure cord felt uncomfortable than cord with15%ALCL3(P<0.01).(5)Compared with two cords the YAJING gingival retraction paste have greater hemostatic and patients’satisfaction(P<0.05).[Conclusion](1)A11the three methods caused the transient gingival irration.the inflammation indexes of paste(YAJING) basically the same as pure cord but significantly lower than cord with15%ALCL3.The pure cord caused acute injury,which also recovered rapidly,the cord with15%ALCL3also caused acute injury,but it recovered slowly,the YAJING paste showed a slighter damage,and slow recovery.(3)Compared with the two cords from measuring the sulcus width,the YAJING paste were greater.(4)The hemostatic and patients’satisfaction of the paste(YAJING) was greater than two cords.(5)The YAJING paste caused the transient gingival irritation,which could recover in fourteen days and used easily,cheeper than imported materials, significient clinical effect,because of all the advantages,it should be promoted in clinic.
