Objective Extralevator abdominoperineal resection (ELAPR) for low rectal cancer has been adopted by centers to improve oncological outcome. The present study aimed to compare intraoperative perforation,blood loss,postoperative complications, operative time and length of hospital stay of ELAPR and conventional APR in our institution.Methods Consecutive patients with low rectal cancer who undergoing APR or EL APE from The First Affiliated Hospital of Kunming Medical University between March2008and September2012were included.They were divided into two group of69patients in extralevator and standard APR. Data were collected from hospital records.Results ELAPR is associated with less intraoperative perforation(p=0.032),blood loss (p=0.001) postoperative complications (p=0.006) and Hospital stay (p=0.001) than APR. No difference in operative time(p=0.105).Conclusion The reseach show some advantages for ELAPE than APR on intraoperative perforation,blood loss,postoperative complications and Hospital stay These results have led to calls for the widely application of this approach.
