Objective: To analyze12-Lead Electrocardiogram with left main coronaryartery disease,To explore the clinical features of Electrocardiogram with left maincoronary artery disease, and to analyze the relationship between Left main coronaryartery disease in patients with ST-segment different amplitude changes in lead aVR ofECG and the severity of stenosis.Methods: From September2010to September2011,patients who werediagnosed in the People’s Hospital of Liaoning province due to coronary heart diseaseassociated with Left main coronary artery disease were selected. The features of12-Lead Electrocardigram were analyze retrospectively. Patients were groupedaccording to lead aVR elevation level will be divided into three groups: Group A:ST-segment elevation in lead aVR≥0.1mm; Group B: ST-segment elevation in leadaVR≥0.05mm and<0.1mm;Group C: ST-segment no change in lead aVR. Compasionthe basic information of groups A,B,C (include age gender, acute myocardial infarction,unstable angina, smoking history, history of hypertension, diabetes, dyslipidemiahistory cases) have no difference. Further comparative analysis of A、B、C three groupaelectrocardiographic characteristics and degree of stenosis; to explore the relationship ofstenosis degree and electrocardiographic manifestation.Results:104cases of Left main coronary artery disease Mean age(65.7±10.1)years, range29~86years,69male,35female, acute myocardial infarction35cases (33.7%), in the group A accounted for9cases, unstable angina63cases,5cases is pure Left main disease, Left main disease associated with multivessel disease in65cases (62.5%).67cases of LM stenosis usually located in the distal bifurcation.Coronary artery stenosis level Ⅰ45cases, level Ⅱ47cases, level Ⅲ10cases, level Ⅳ2cases.16cases is nomal resting electrocardiogram, all for the group C, in which acute non-ST-segment elevation myocardial infarction1cases, unstable angina1cases,description of patients with Left main disease electrocardiogram can demonstratenormal, even when a serious condition, can also be no abnormal electrocardiogram. Thecomparison of A、B、C three groups in age, gender, acute myocardial infarction,unstable angina, smoking history, primary hypertension, dyslipidemia, diabetes historythere were no statistically significant differences, but in the stenosis degree obviouslydifferent, group A>group B>group C, tip aVR ST-segment elevation than lead aVRST-segment no offset stenosis degree, and lead aVR ST-segment elevation moreapparent, stenosis degree is bigger, the more malignant.Patients with Left main diseaseof12-leads ECG as the main performance: ST-segment elevation in leads aVR、V1andaVR ST-segment depression in leadsⅠ、Ⅱ、V4~V6;group A of12-leads ECG as themain performance: ST-segment elevation in leads aVR、V1、V2and ST-segmentdepression in leadsⅡ、Ⅲ、aVF, the average degree of stenosis as (83.33±14.4)%; groupB of12-leads ECG as the main performance: ST-segment elevation in leads aVR、V1and aVR ST-segment depression in leadsⅠ、Ⅱ、V4~V6,the average degree of stenosisas (76.67±10.6)%; group C of12-leads ECG without specific performance, the averagedegree of stenosis as (61.29±11.18)%.Conclusion:1、Patients with left main disease of12-leads ECG as the mainperformance: ST-segment elevation in leads aVR、V1and aVR ST-segment depressionin leadsⅠ、Ⅱ、V4~V6;2、when the ST-segment elevation of lead aVR≥0.1mm, and ST-segment elevationin leads aVR、V1、V2and ST-segment depression in leadsⅡ、Ⅲ、aVF, prompting thestenotic lesion degree of (83.33+14.4)%; when the ST-segment elevation of lead aVR≥0.05mm and<0.1mm,with ST-segment elevation in leads aVR、V1and aVR ST-segment depression in leadsⅠ、Ⅱ、V4~V6, prompting the stenotic lesion degree of(76.67±10.6)%.3、 ST-segment elevation of lead aVR greater tip lesion range is also larger, highermalignant degree.4、 left main coronary artery disease may manifest as normal ECG...
