Objective To investigate the value of ST segment changes of aVR lead for predictingthe infarction related artery(IRA) and evaluating the short-term prognosis in acutemyocardial infarction.Methods According to the ST segment changes of aVR lead,100selected patientsdiagnosed with STEM,were divided into ST segment elevation of aVR lead(aVRSTE)group(31cases),ST segment without deviation(aVRSTWD) group(59cases) and STsegment depression(aVRSTD) group(10cases).Three groups were compared thedifference with gender,age,the concentrations of CTnI,CK-MB in the peak,theconcentration of uric acid,hypertension or diabetes,left ventricular ejectionfractions(LVEF) and left ventricular end diastolic dimension(LVEDd). The study is toanalysis the relationship between the incidence of IRA,the counts of coronary arterylesion and the incidence of heart attacks with ST segment changes of aVR lead in thethree groups.Results:1.The concentration of CTnI,CK-MB in the peak of aVRSTE group issignificantly higher than aVRSTWD group(P<0.01);2.The incidence of IRA withmore teams(≥3)lesions,especially combined LM, LAD, LCX lesions in the aVRSTEgroup at the same time,is significantly higher than aVRSTWD and aVRSTD group(P<0.05),and aVRSTE have stronger predictive value of IRA;3.aVRSTE suggests ahigh incidence of cardiac adverse events in the short term and a poorer prognosisafter STEMI.Conclusion: In patients with STEMI,AVRSTE has strong predictive value ofIRA,diagnosed with multivessel lesions(more teams(≥3),especially combined LM,LAD, LCX lesions at the same time;aVRSTE suggests a high incidence of cardiacadverse events in the short term and a poor prognosis.
