ObjectiveTo compare the effection of treatment and prognosis by three operative approacheswith open beating heart surgery for repairing atrial septal defect.Method51patients were treated with three different operative approaches:17patientsunderwent right axillary minithoracotomy;19patients underwent thoracoscopy; and15patients underwent median sternotomy operation. The thoracoscopy operation group usedfemoral artery and vein cannulation to establish CPB; Right arillary minithoracotomygroup used ascending aorta and right angle vena cava cannulation to establish CPB;Median sternotomy group used regular cannulation to establish CPB.ResultOperations were successful and all patients recovered well. Comparing to mediansternotomy, the thoracoscopy operation group has advantages including minithoracotomy,no need to wound sternum, less hemorrhage and drainage, and shorter hospitalization days(P <0.05). There were also differences in bypass time and operation time, but no statisticdifference;there were differences in trauma, chestmalformation, hemorrhage, drainagebetween right arillaryminithoracotomy and regularmedian sternotomy (P <0.05), but nostatistic difference in hosp italization days.ConclusionsThough the ways of cardiopulmonary bypass by three operative approaches with openbeating heart surgerywere different, the management of cardiopulmonary bypass was nosignificant difference. The three operative approaches were all safe and technically feasiblebut minimal invasive approach was better than median sternotomy.
