Objective To explore the characteristics, etiology and risk factors of chronic severehepatitis B with nosocomial infection.Methods275cases of chronic severe hepatitis B, not treated by antibiotic for1week before admission, no signs of infection on admission and hospitalized for morethan72hours, were collected from January2008to January2012in the first hospitalof Quanzhou affiliated with Fujian Medical University and grouped according towhether nosocomial infection or not.Results The incidence rate of nosocomial infection in chronic severe hepatitis Bpatients with nosocomial infection was25.45%. Nosocomial infection occurredfrequently in the abdominal cavity (43.56%), respiratory tract (30.69%) and urinarytract (6.94%) in turn. Age, longer hospital stay, prophylactic using broad-spectrumantibiotics or taking more than two antibiotics and glucocorticoids for long time,receiving invasive operation, hypoalbuminemia and diabetes were risk factors.Thepathogens of nosocomial infection were mainly Gram-negative and Gram-positivebacteria. Fungal infections also occupy a certain proportion.Conclusion The incidence rate of nosocomial infection in chronic severe hepatitis Bwas high. The primary site of nosocomial infection were in the abdominal cavity andrespiratory tract, and the main pathogens were gram-negative and Gram-positivebacteria. longer hospitallization time, using broad-spectrum antibiotics or taking morethan two antibiotics and glucocorticoids for long time, receiving invasive operation,hypoalbuminemia and diabetes were risk factors.
