Object:Radical prostatectomy postoperative Gleason score of3+4and4+3twogroups of clinical data analysis to explore the difference of two groups of clinicalfeatures and PSA recurrence,and analyzes the influencing factors of PSA recurrence.Medthods: The clinical records of patients who underwent radical retropubicprostatectomy in our hospital from January2008to December2011wereretrospectively analyzed. Fifty-one patients with a Gleason score of7after operationwere divided into two groups, Gleason score3+4group and Gleason score4+3group. Kaplan-Meier method was used to compare the progression-free survivalcurves, the log-rank test single factor analysis was used to screen prognostic factorsaffecting radical prostatectomy.It further classified independent prognosis factors inthe Cox proportional risk regression multivariate analysis and compare the prognosisof the two groupsResults:1.Initially screened by univariate analysis the prognosis after radicalprostatectomy influential six factors: the rates of extraprostatic extension, seminalvesicle invasion, surgical margin positive rates, GS7typing, clinicalstages,endocrine pharmacotherapy. Patient’s age, preoperative serum PSA, prostatevolume,surgical approach, the surgeon, five factors were excluded because of P>0.05. Cox regression model showed that GS7typing, rates of extraprostatic extension,surgical margin status radical postoperative independent prognostic factors affectprostate cancer was independent factors of prognosis (P=0.020, P=0.017,P=0.027).2.There was statistical difference in the progression-free survival curvesbetween GS3+4and GS4+3groups (P=0.026). The non-progression survival timeof GS3+4group was markedly longer than that of GS4+3group. Extraprostaticextension and surgical margin status were stratified and compare prognosis of GS3+4group and GS4+3group, there was significant difference in the prognosis of thetwo groups of patients with positive surgical margins.Conclusions:1.The factors of influencing PSA recurrence:the rates of extraprostatic extension, seminal vesicle invasion, surgical margin positive rates, GS7typing,clinical stages; surgical margin status radical postoperative independent prognosticfactors affect prostate cancer was independent factors of PSA recurrence.2.In the GS7patients, GS4+3group had a higher risk of biochemical recurrence than GS3+4group with positive surgical margins.
