| Purpose The study compares the application evaluation of the bedside index for severityin acute pancreatitis (BISAP) and acute physiology and chronic health evaluationⅡ(APACHEⅡ) scoring systems in early prediction of severity and prognosis in acutepancreatitis (AP).Methods This study retrospectively analysis clinical data from203AP cases of theGeneral Hospital of Ningxia Medical University collected during January2010andDecember2011. Scores for all the AP patients are computed using BISAP and APACHEⅡmethods. By analyzing the receiveroperating characteristic curve and the area under the curve(AUC) comparing the abilities to predict the severity and prognosis of AP between thebedside index for severity in acute pancreatitis (BISAP) and acute physiology and chronichealth evaluation Ⅱ (APACHEⅡ) scoring systems.Results Of203AP patients cases,there are114cases diagnosed mild acute pancreatitis(MAP),89cases diagnosed severe acute pancreatitis (SAP),194cases alive and9cases died.The greater the BISAP score,the higher proportion of severe acute pancreatitis and deathwere. These two scoring systems were significantly related with patients’ hospitalized timeand fasting time. The differences between BISAP and APACHEⅡscoring systems werestatistically significant across SAP and MAP,with/without organ failures,with/without death(P<0.001). APACHEⅡ scoring system has advantages on predicting the illness severity,systemic complications and organ failure,the AUC areas respectively were0.906,0.939, 0.932. It has statistically difference compared with BISAP score system (P<0.05). Besides,it was similar to BISAP score in predicting local complications.Conclusion BISAP and APACHEⅡ scoring systems had significant difference in theseverity of disease,with/without organ failures,with/without death. The higher the score,the heavier the illness were,and the higher organ failure and mortality were。APACHEⅡscore are stronger than BISAP score in predicting the severity of AP,systemic complicationsand the ability of organ failure. But because of BISAP score’s simple,effective,rapid andpractical characteristics,it can effectively predict the severity and prognosis of AP,carriedout in grassroots hospital and in preliminary screening for SAP. |
