Objective:This study through the detection of diabetic foot ulcers in patients with different TCM syndrome types of Ankle brachial pressure indices, laser doppler perfusion percentage, matrix metalloproteinase-2and matrix metalloproteinase-9indicators of change, is discussed with the correlation of traditional Chinese medicine, thus for clinical diagnosis and treatment of traditional Chinese medicine objective and provides a more objective basis.Method:This research adopts the prospective stratified random single blind method, collected60cases of patients with diabetic foot ulcers, according to the classification standard of syndrome differentiation can be divided into qi deficiency blood stasis type of30cases,30cases of damp and hot accumulate knot type, and22patients with non-diabetic controls. Using enzyme-linked immunosorbent determination PBLS MMP-2and MMP-9, and detection of ankle brachial pressure indices, laser doppler perfusion, inflammation related factors, changes in the indexes of blood lipids, blood glucose. Relevant data statistics by comparative analysis and regression analysis.Result:In inflammation related indicators, diabetic foot group compared with control group, significantly lower ESR, CRP damp and hot accumulate knot group is significantly higher than the control group; ESR, CRP, qi deficiency and blood stasis group was lower than that in group damp and hot accumulate knot. In lipid related indicators, CHOL, qi deficiency blood stasis group is significantly higher than the control group, LDL-C damp and hot accumulate knot group is significantly higher than the control group, LDL-C qi deficiency blood stasis group is significantly lower than the control group, HDL-C group damp and hot accumulate knot was lower than that in control group; CHOL, LDL-C damp and hot accumulate knot group obviously higher than that of qi deficiency blood stasis, HDL-C damp and hot accumulate knot below the qi deficiency blood stasis group.In terms of glycemic index, FPG,2HFPG, HBA1C were significantly higher than the control group. In terms of blood microcirculation index ABI and laser doppler perfusion percentage were significantly higher than the control group. MMP-2significantly lower than the control group, MMP-9damp and hot accumulate knot group is significantly higher than the control group; MMP-2qi deficiency and blood stasis group obviously higher than that of damp and hot accumulate knot, MMP-9qi deficiency and blood stasis group was lower than that in group damp and hot accumulate knot. By multiple linear stepwise regression analysis of MMP-2and MMP-9relationships with more observation index found that MMP-2percentage changes with the laser doppler perfusion, ESR and FPG with linear regression relations, MMP-9with the change of CRP, ESR and duration of diabetes have linear regression relation.Conclusion:Patients with diabetic foot ulcer of traditional Chinese medicine, blood glucose, blood lipids, inflammatory reaction and ankle brachial percentage ratio and laser doppler perfusion have different degree of correlation.Diabetic foot patients with damp and hot accumulate knot type dynamic plasma sedimentation rate and c-reactive protein increased significantly than normal qi deficiency and blood stasis patients compared with less obvious inflammation reaction. Diabetic foot ulcers compared with the blood lipid in patients with diabetes, mainly reflects in cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, the difference between cholesterol is low, qi deficiency and blood stasis group and damp and hot accumulate knot group and low density lipoprotein cholesterol were higher. Glycemic control in patients with diabetic foot ulcer overall level is poor, and there are different degree of ischemia, qi deficiency and blood stasis type of ischemia degree is more obvious.Matrix metalloproteinases2biggest correlation with laser doppler blood flow perfusion percentage. Matrix metalloproteinases9correlation between c-reactive protein and is the largest.
