ObjectiveTo investigate the different expression of plasma M type phospholipase A2receptor (PLA2R) antibody inidiopathic membranous nephropathy (IMN) and Hepatitis B virus associated membranous nephropathy (HBV-MN), and the significance of plasma M type PLA2R antibody in differential diagnosis of HBV-MN.MethodsPlasma samples were obtained from patients with IMN, HBV-MN and minimal change disease (MCD).respectively, before immunosuppressive therapy. Concentrations of plasma M type PLA2R antibody were detected by sandwich ELISA.ResultsConcentrations of plasma M type PLA2R antibody were15.4±7.2ug/ml in IMN group,10.3±5.7ug/ml in HBV-MN group (P<0.01, between IMN group and HBV-MN group), and all less than0. lug/ml in MCD group respectively. If raise the concentration of plasma M type PLA2R antibody diagnosis threshold value up to10.3ug/ml (the average concentration of plasma M type PLA2R antibody in HBV-MN group),7cases in IMN group were positive (70%);7cases in HBV-MN group (28%)ConclusionsConcentrations of plasma M type PLA2R antibody were significantly higher in IMN group than in HBV-MN group. If raise the concentration of plasma M type PLA2R antibody diagnosis threshold value up to10.3ug/ml,7cases in IMN group were positive (70%);7cases in HBV-MN group (28%). The result shows that there is obvious clinical significance of concentrations of plasma M type PLA2R antibody in diagnosis of IMN and in differential diagnosis of IMN and HBV-MN.
