Objective:To observe the clinical efficacy of the treatment of calming the liver、 tonifying the yin fluid and clearing the internal heat in treating dry eye.Methods:This study reviews the clinical data of60patients (120eyes) with dry eye. Sixty cases of the patients were divided into group A and B. Thirty cases (60eyes) in group A were treated with the treatment of calming the liver tonifying the yin fluid and clearing the internal heat and topical artificial tears(Sodium Hyaluronate Eye Drops). Thirty cases (60eyes) were treated only with topical artificial tears(Sodium Hyaluronate Eye Drops) in group B. After the treatment, the efficacy in both groups was observed. With Group A, the improvement in the quantity of tears、the tear break-up time (BUT)、the corneal fluorescein staining, and the symptom score(including dry and rough of eye、 the felling of a foreign body in the eye、pain of eye、visual fatigue、red eye) after therapy were observed. This study also made a comparison on the improvement in the quantity of tears、the tear break-up time (BUT)、the corneal fluorescein staining and the symptom score(including dry and rough of eye、the felling of a foreign body in the eye、pain of eye、visual fatigue、red eye) after treatment with group A and B. During the study, the patients who had high blood pressure、 diabetes、coronary heat disease and other disease of medical department were still under their original therapy. And patients with followed situations were ruled out at the beginning of the study:those who had other eye disease like pathological changes of conjunctiva and cornea, scar on conjunctiva of eyelid; those who had stenosis of lacrimal passage; those who had eye surgery in6months before the study; those who had disease of whole body for example Sjogrens syndrome; those who had severe primary disease of heat、cerebrovascular、liver and the system to make blood; those who had mental illness; those who were suspicious or confirmed of drug abuse. Patients in Group A were critically selected and they were all yin-deficiency and hyperactivity of the liver-Yang syndrome. Thirty cases(60eyes) in group A were treated with the treatment of calming the liver、tonifying the yin fluid and clearing the internal heat(decoction formulated by myself and bear gall capsule) and topical artificial tears. The decoction formulated by myself is made up of: Gypsuml5g, Abalone shell or sea-ear shell30g, Puncture vinelOg, Uncaria stem with hooks10g, Chrysanthemum Flower10g, Scutellaria Root10g, Moutan BarklOg, Flos Eriocauli10g, Flos Buddlejae10g, Herba Equiseti Hiemalis10g, Dendrobium Stem20g, Wolfberry Fruit12g, Flatstem Milkvetch Seed10g, Kudzuvine Root6g, Antelope’s Horn0.6g,Medicated Leaven10g, For treatment, the patient should take one dose per day, in the morning and evening. The therapy lasted for8weeks. Thirty cases (60eyes) were treated only with topical artificial tears in group B.Results:The effective rate was76.67%in group A, and in group B it was56.67%. The difference between the two groups was significant(P<0.05). The quantity of tears、the tear break-up time (BUT). the corneal fluorescein staining and the symptom score were extremely improved in group A after treatment (P<0.01). And compared with group B, there was an extreme significant difference in efficacy (P<0.01).Conclusion:the treatment of calming the liver、tonifying the yin fluid and clearing the internal heat is effective in the treatment of dry eye. many patients of dry eye I treated are belong to the syndrome of hyperactivity of the liver-yang caused by deficiency of yin-fluid.These patients are deficient of liver and kidney-yin. They also have hyperactivity of the liver-Yang and fire in the liver. The fire in the liver can reduce kidney-yin, so kidney-yin become more deficient.Kong Bohua, one of the four most renowned doctors in Beijing, held that constant excess of yang generates fire, and constant deficiency of yin creates heat; the difference is that one is resulted from excess and the other deficiency. If combined with other fire-causing factors such as mind activities and emotional disturbance, an environment of heat-plus-fire inside the body thus comes into shape where heat-plus-fire type of conditions tend to develop. As people are living an increasingly stressful life, they are easy to have stagnation in the liver caused by anxiety, annoyance, depression, etc. Moreover, frequent intake of food that is spicy, fatty, or with strong flavors tends to produce heat inside the body which transforms liver stagnation into strong fire in the liver as well as relating structures. As a result, heat-plus-fire develops when heat generated from yin deficiency in the liver and kidney and fire in the liver intertwine. Fire is a type of yang pathogen which likes to flame upward by nature. The eyes, which are at the upper body, are vulnerable to the fire flame so that dryness in the eye tend to happen. Self-prescribed formula plus Bear Gall Capsule is effective in treating dryness in the eye as it deals with the condition at comprehensive levels which tonifies kidney-yin, tranquilizes liver-yang and dissipating liver-heat at the same time.
