Research objective:1.To discuss about the difference between IgA nephropathy patients and the healthy matched group in the degree of atherosclerosis;2.To analyze of the risk factors of IgA nephropathy patients’ atherosclerosis degree;3.To explore the Chinese medicine dialectical typing of IgA nephropathy patients and the relativity with atherosclerosis.Research methods: Analyzed40IgA nephropathy patients who hospitalized in the China-Japan Friendship Hospital, and the40healthy cases were took as the control group trough examination. Hypertension, diabetes. coronary heart disease and dyslipidemia were all excluded. Recorded the ankle-brachial index(ABI) and brachial-ankle pulse wave velocity(baPWV) of the two groups. Then recorded the case group’s gender, age. height, weight. creatinine(CR). blood urea nitrogen(BUN), serum uric acid(SUA), glomerular filtration rate (GFR), creatinine clearance rate(CCR), fasting blood glucose (FBG),24-hour urinary protein excretion, triglyceride (TG), total cholesterol (TC). high density lipoprotein (F1DL). low density lipoprotein (LDL), systolic pressure(SBP). diastolic pressure(DBP) for statistic analysis.Research results: The baPWV of IgA nephropathy patients is significantly higher than that in normal. Bivariate correlation analysis of Clinical biochemical indexes and arteriosclerosis showed: the baPWV were correlated with CR, BUN. SUA, CCR, GFR,24hour urine protein. FBG. SBP, DBP and the age: the right ABI were correlated with BUN.24hour urine protein. TG. HDL. GFR and the age; the left ABI were correlated with CR. BUN. CCR. GFR. SBP and the age. Multiple linear regression analysis showed that in addition to traditional risk factors such as age and hypertension. SUA. FBG. CR were the main risk factors of baPWV; CR and TG were the main risk factors of ABI. Canonical correlation analysis of arteriosclerosis indexes and the kidney indexes showed that baPWV. ABI were correlated with CR, BUN, SUA, GFR, Ccr,24hour urine protein. In addition, baPWV were mostly related to SUA, CR and24hour urine protein; ABI were mostly related to CR and GFR. The frequency of TCM Syndrome were Qi and Yin deficiency. Yin deficiency of liver and kidney, Yang deficiency of spleen and kidney, Qi deficiency of spleen and kidney. Multiple linear regression analysis of classified variable showed that the numerical size of right baPWV were Yang deficiency of spleen and kidney>Qi and Yin deficiency>Yin deficiency of liver and kidney>Qi deficiency of spleen and kidney; the numerical size of left baPWV were Yang deficiency of spleen and kidneys>Yin deficiency of liver and kidney>Qi and Yin deficiency>Qi deficiency of spleen and kidney; the numerical size of right ABI were Yin deficiency of liver and kidney>Qi deficiency of spleen and kidney>Qi and Yin deficiency>Yang deficiency of spleen and kidney; the numerical size of left ABI were Yin deficiency of liver and kidney>Yang deficiency of spleen and kidney>Qi and Yin deficiency>Qi deficiency of spleen and kidney.Research conclusions:1. The degree of atherosclerosis in patients with IgA nephropathy is significantly higher than the matched group.2. In addition to traditional risk factors such as hypertension, high cholesterol, senior citizens, proteinuria, CR, BUN, SUA. Ccr,24-hour urinary protein, GFR and other non-traditional risk factors played an important role on atherosclerosis in patients with IgA nephropalhy. indicated that kidney indexes were independent factors changes of arterial stiffness.3. The risk factor of atherosclerosis is not independent existence, hut various risk factors work together to promote atherosclerosis.4. Chinese sub-type of IgA nephropathy patients. Qi and yin deficiency type is the most. Spleen and kidney yang deficiency syndrome has the highest baPWV value. and Yin deficiency of liver and kidnev has the highest ABI value.
