Based On Factor Analysis Of Gout Disease Pathogenesis Research

ObjectiveTo discuss the distribution of syndrome elements of hyperuricemia, urarthritis and diapause, which are the major components of gout. To conclude the disease mechanism of gout, to provide the basis for rules and measures of TCM treatment of gout, as well as early intervention.MethodsThe research had collected100patients of asymptomatic hyperuricemia,69patients of urarthritis and72patients of diapause of gout. All the subjects were investigated by four diagnostic information of TCM. Frequency analysis was adopted for the distribution of TCM symptoms; while factor analysis was adopted for the distribution of syndrome elements.ResultsThe highest frequency of TCM symptoms of asymptomatic hyperuricemia are spontaneous perspiration(59%), dysphoria and irritability(52%), thirst(49%), feeling sleepy(49%), aversion to hot(45%), yellow and hot urine (45%), sore waist (44%), fatigue (43%), tasteless sense of mouth (42%), dizziness and heavy-headedness(38%). The major syndrome elements of asymptomatic hyperuricemia are dampness heat syndrome(36%),yin deficiency syndrome(30%), phlegm dampness syndrome(19%),qi deficiency syndrome(9%),yang deficiency syndrome(6%).The highest frequency of TCM symptoms of urarthritis are yellow and hot urine(83%),the red of gouty articulation (84%), swelling of gouty articulation (83%),heat of gouty articulation(74%), high temperature of gouty articulation(72%),bitter taste (71%),thirst(64%), insomnia(57%), stabbing pain of gouty articulation (54%), much nocturnal urine (54%). The major syndrome elements of asymptomatic urarthritis are phlegm dampness syndrome(29%), dampness heat syndrome(26.1%), yin deficiency syndrome(23.2%), yang deficiency syndrome (11.6%), blood stasis syndrome(7.2%) and qi deficiency syndrome(2.9%).The highest frequency of TCM symptoms of diapause are dysphoria and irritability (76%), yellow and hot urine (67%), fatigue (54%), much nocturnal urine(54%),dizziness and heavy-headedness(49%), thirst (49%), insomnia (46%), abdominal distension(46%), leg heaviness(44%).aversion to cold of arms and legs(43%),amnesia(43%).The major syndrome elements of diapause arc yin deficiency syndrome(36.1%),dampness heat syndrome(23.6%),yang deficiency syndrome(18.1%), phlegm dampness syndrome(15.3%), qi deficiency syndrome(6.9%).There are differences of phlegm dampness syndrome (X2=5.711, P=0.017) and yin deficiency syndrome (X2=4.063, P=0.044) between urarthritis and dysphoria. There is difference of yang deficiency syndrome(X2=6.818, P=0.009) between asymptomatic hyperuricemia and dysphoria.ConclusionGout is deficiency-excess mixing。The major syndromes of asymptomatic hyperuricemia are phlegm dampness syndrome and dampness heat syndrome。 The major syndromes of uarthritis are phlegm, dampness, hot and blood stasis. The major syndromes of dysphoria are yin deficiency and yang deficiency. The major syndromes of early and acute stage of gout are phlegm, dampness and hot. In course of time,the major syndromes turn to yin deficiency and yang deficiency.
