Objective:Comparative analysis of low-dose64-slice spiral CT scan and target sweep the advantages and disadvantages of early stage lung with traditional DR chest X-ray screening, evaluation of low-dose64-slice spiral CT scan and target scan in early lung cancer screening。Method:1. The program uses the Philips64row level128ultra-high-end nano-CT Brilliance conventionally used (30mAs,120kv),512×512layers lung scan found a small lesions and then (1024×1024matrix, FOV15cm, thickness<1mm) superhigh-resolution local target sweep。2. The chest X-ray film using the target population of high-risk groups and the general population grouping, First step, The orthotopic lung slices target population DR radiography, and recorded positive results; The second step, Low-dose64-slice spiral CT scan, patients with suspicious nodules further target sweep to obtain high-quality CT images of the target population; The third step, Two radiology physician to observe the CT and DR-chip double-blind method, to form a unified opinion stated in the results, the opinions inconsistent adopted another superior physician advice. Follow the lesion classification statistics (Overlap in the statistics:if lung disease may also have pleural lesions and fibers of the cable video were1times each in the two groups), Statistics of positive cases.3. The study from October2010to March2013, Hunan University of Traditional Chinese Medicine, health examination center attached to a hospital to participate in the examination crowd. Inclusion criteria:age more than40. Lung cancer high-risk groups into the standard:Smoking history (starts smoking aged more than20, more than20years smoking and smoking more than300cigarettes years; or more than20cigarettes a day, and continuous smoking for more than20years); History of lung cancer; History of dust exposure; previous history of lung disease: chronic bronchitis, pulmonary tuberculosis。Exclusion criteria:one of the following persons not enrolled:age less than40years; been diagnosed with lung cancer patients; other parts of the tumor existing pulmonary metastases; history of lung surgery. In line with high-risk conditions for a total of512people,283men and229women, aged40to82years, with an average age of55, including smoking history of392people (389men,3women)4. Ratio or rate difference of statistical methods for inspection. Test, exact method to a0.05level of inspection。Results:1.64row spiral CT screening high-risk groups:512cases pulmonary lung nodules number is82, while the DR radiography nodules in the crowd of positive number is36, the difference was statistically significant (P<0.05).2. Screening of5mm nodules. CT36and DR,7;5mm~10mm were screened for CT24, DR to16; screening10mm more nodules in CT21, and DR13, the difference was statistically significant (P<0.05)。3.The64row spiral CT screening of12patients with suspected malignant nodules, then target sweep after the exclusion of3cases, high9highly suspected malignant nodules, the last7cases confirmed lung cancer, the other2cases were benign lesions; DR screened7cases of malignant nodules, finally confirmed3cases of early lung cancer, and the other4cases for the benign lesions, the difference was statistically significant (P <0.05).Conclusion:1.Capacity of1.64row spiral LDCT scanning and target high-risk populations for screening early lung cancer was significantly higher than that of DR. The64row spiral LDCT screening for lung nodules, especially small nodules is obviously better than that of DR。2.This suggests that we should pay attention to LDCT in the practical work, screening for lung cancer at high risk should be preferred to LDCT, rather than the commonly used DR。...
