Objective To analyze survival difference among various models of HBV serological markers in HCC patients underwent RO resection, and investigate the relationship between the poor prognosis in the models of HBV serological markers and the clinical and pathologic factors.Methods Preoperative clinical, oncologic data and HBV serological markers (HBsAg, HBsAb, HBeAg, HBeAb and HBcAb) were retrospectively analyzed in the343patients with HCC related with HBV infection who underwent RO resection from February2002to December2008at the Affiliated Hospital of Medical College Qingdao University. These patients’ prognosis was evaluated.Results Fifteen kind of models of HBV serological markers were found in343patients with HCC. The first common model was the HBsAg, HBeAb and HBcAb positive patients (175,51.3%), the second was both HBsAg and HBcAb positive (86cases,25.1%), the third was the HBsAg, HBeAg and HBcAb positive patients (40,11.7%), and other patients (42,12.0%). By Kaplan-Meier analysis, we found the median overall survival (OS) within theses models was69.0.27.9、84.0and72.0month, the disease-free survival (DFS) was32.2.17.8、84.0and42.0month; the OS and DFS in HCC patients with the both HBsAg and HBcAb positive model was poor than the neither models (Log Rank test, P<0.05). The patient’s age≤60y, non-integrity of the tumor capsule and with vesicular invasion in the HBsAg and HBcAb positive model was more than other models by χ2test. For the patients with HBsAg and HBcAb positive model, the Kaplan-Meier analysis and Cox regression model showed that the Child-Pugh classification, tumor size and integrity of tumor capsule was the independent risk factors for OS, and the integrity of tumor capsule was an independent risk factor for DFS.Conclusion HBV infection models are intensely related to HCC patients’ survival and recurrence after hepatorecetion. The prognosis of HCC patients underwent R0resection with the serologic HBsAg and HBcAb positive models are poor. Child-Pugh classification, tumor size and integrity of tumor capsule was the independent risk factors for OS, and the integrity of tumor capsule was an independent risk factor for DFS.
