Backgroud and Objective:Gallbladder carcinoma is highly malignant tumor of digestive system, which incidence rate is at the top of the biliary tumor. Because of the covert incidence and lacking of clinical symptoms and signs, gallbladder carcinoma is difficult to diagnose. When the majority of the patients seek treatment, the cancer cells have Local invasion or distant metastasis. The resection rate and5-year survival rate of gallbladder carcinoma are very low. Therefore, to research the mechanism of invasion and metastasis of gallbladder carcinoma and explore the early diagnostic markers and prognostic indicators of gallbladder carcinoma is one of the hot spots in current research.CD147is a cell adhesion molecules, which mediate intercellular adhesion. At the same time, as the matrix metalloproteinase inducer, CD147can stimulate tumor cell and fibroblast to synthetize matrix metalloproteinase. MMP-9is gelatinase B, It can degrade type â…£ã€Vcollagen and cause destruction of the basement membrane integrity to promote tumor invasion and metastasis. At present, the study of CD147and MMP-9in cancer has been reported. But the studies of two in gallbladder carcinoma are rare at home and abroad. We tested the expression of CD147and MMP-9in42gallbladder carcinoma,18gallbladder epithelial atypical dysplasia â…¡-â…¢ tissues,13gallbladder epithelial atypical dysplasia I tissues,15gallbladder adenoma tissues and15chronic cholecystitis tissues with paraffin blocks. Combined with clinical data were analyzed, so as to explore the relationship between them and the gallbladder carcinoma occurrence, progression and prognosis.Materials and methods:1. The samples of paraffin blocks for research from the pathological Department of the First Affiliated Hospital of Zhengzhou University, the Second Affiliated Hospital of Zhengzhou University and Affiliated Tumor Hospital of Zhengzhou University between January2005to June2012, and42gallbladder carcinoma from January2005to January2007. SP immunohistochemistry was used to determine the expression of CD147and MMP-9in surgically resected specimens of42gallbladder carcinoma,18gallbladder epithelial atypical dysplasia â…¡-â…¢ tissues,13gallbladder epithelial atypical dysplasia I tissues,15gallbladder adenoma tissues and chronic cholecystitis tissues.2. Application of SPSS17.0software for statistical processing of data. χ2test was used to compare categorical data. Spearman rank correlation test for the correlation between CD147and MMP-9. The prognosis of postoperative was done by Kaplan-Meier method survival analysis, and with the log-rank test. Pvalue<0.05was considered as statistieally significant Value.Results:1. The positive rate of CD147and MMP-9proteins (83.33%,88.10%) w ere higher in gallbladder carcinomas than gallbladder epithelial atypical dysplasi a I tissues,(38.46%,30.77%), gallbladder adenoma (20.00%,26.67%) and chronic cholecystitis (6.67%,20.00%)(P<0.01), but no significant difference c ompared to gallbladder epithelial atypical dysplasiaâ…¡-â…¢ tissues,(61.11%,66.67%P>0.05)2. Expressions of CD147and MMP-9were positively correlated with the tumor differentiation, Nevin staging and Lymph node Metastasis (P<0.05). CD147expression was positively correlated with MMP-9expression in gallblad der carcinoma (rs=0.427,P=0.005).3. Patients with negative expression of CD147and MMP-9had longer median survival time and average survival time than those with positive expression of CD147and MMP-9.Conclusion:1. The expression of CD147and MMP-9in gallbladder carcinoma were positively correlated with the tumor differentiation, Nevin staging, Lymph node Metastasis and postoperative survival.2. CD147may control the invasion and metastasis of gallbladder carcinoma by stimulating the expression of MMP-9.. |
