Effects Of Diabetes Distress On Quality Of Life In Patients With Type2Diabetes

Objective:To describe the present status of diabetes distress in patients with type2diabetes, and to analyze its influence factors; to explore the relationship between diabetes distress and depression; to investigate the general condition of patients’quality of life (QOL) and to explore the effects of diabetes distress and depression on QOL in patients with type2diabetes.Methods:A cross-sectional study was conducted, and a convenience sample was received.216patients with type2diabetes were recruited during December2011to July2012from2grade-A tertiary hospitals in Shandong, China. Participants completed General Information Questionnaire, Diabetes Distress Scale (DDS), Self-rating Depression Scale (SDS), and Adjusted Diabetes Quality of Life Measure (A-DQOL). The collected data was analyzed by SPSS17.0.Results:1.Among the200patients with type2diabetes, the incidence of diabetes distress was64%, and the score of each dimension were emotional burden (2.89±0.96) regimen-related distress (2.42±0.80), interpersonal distress(2.22±1.06), and physician-related distress(1.72±0.88), respectively. 2. By the one-factor analysis and two-factors Logistic Regression, we found those who were male, age<50years, with lower monthly income of per family, with short disease course, had many treatment of diabetes, received treatment of other diseases, with lower confidence in diabetes management, and had poorer disease control, were more easy to have diabetes distress.3. The incidence of depression in our study was26%, which was similar to other reports. Otherwise, the total score of diabetes distress and each dimension were positive related to the score of depression (P<0.05, P<0.01), and the correlation coefficients were0.318、0.159、0.356、0.184、0.299, respectively, besides, the diabetes distress was also the risk factor of depression(OR=4.264,P=0.000).4. The total score of QOL in our study was122.27±26.79, which was moderate level. The score of each dimension were satisfaction(42.23±7.96), influence(49.26+9.96), anxiety Ⅰ(13.52±5.27) and anxiety Ⅱ(10.95±3.93), respectively. The score of satisfaction and anxiety Ⅱ were higher in the total score of dimenion, which indicated that the moderate QOL was rise from the two dimensions.5. The score of diabetes distress was positively related to the total score of QOL and the score of each dimentions. respectively(P<0.01). the correlation coefficients were0.540、0.490、0.439、0.482、0.272,respectively; the scale of depression was positively related to the total score of QOL and the score of satisfaction, influence and anxiety Ⅰ,respectively.(P<0.05,P<0.01), the correlation coefficients were0.218、0.121、0.311、0.144.However, the relationship between depression and anxiety II had no statistical significance.6. There were so many factors influencing patients QOL, and after the one-factor analysis of variance, we found that sex, age. education level,career, monthly income of per family, treatment method, the confidence of diabetes management and disease control could influence patients’ QOL. but when depression and diabetes was recruited in the Multiple Linear Regression, only depression, diabetes distress and disease control entered the equation, which can explain47.1%variations of QOL, and the comparation of standard coefficients indicated that, diabetes distress played a more important part than depression in QOL. Conclusions:1. Diabetes distress was prevalence in patients with type2diabetes, which also indicated that the health education about diabetes patients needs to be promoted.2. Diabetes distress was influenced by sex, age, monthly income of per family, treatment of other disease, treatment method, confidence in diabetes management and disease control, so the health caregivers should pay more attention to the characteristics when intervene.3. Diabetes distress and depression were two different kinds of psychological state, and the occurrence of depression was related to the severity of diabetes distress.4. Diabetes distress and depression were both related to QOL in patients with type2diabetes, and the more severe of diabetes distress, the more severe of depression, the poorer of of patients’QOL.5. The QOL in our study was moderate level, which should be promoted. There were so many factors affected patients’QOL, but diabetes distress, depression and disease control played an more important part in psychological level. So we suggested that the health caregivers pay more attention to diabetes distress, and conduct some primary health care project to prevent, monitor and intervene patients’psychological health.
