Objective The purpose of the study is designed to establish the model of pig withacute heart failure to validate the auxiliary effect of intraaortic counterpulsationdevice(IABP) and paraaortic counterpulsation device (PACD)Method Ten healthy adult panama miniature pig were randomly selected for researchsubjects, weighting (44.5+2.8) kg, nomatter male or female.In IABP study weimplanted a pair of cavity with airbags and pressure sensor probe at the top of the tubeto the descending aorta in the chest through the left/right common iliac artery ofexperimental animals. The half spherical PACD was a piece of circulation assistingdevice consisting of blood capsule and balloon capsule separating by a diaphragm. Thesingle export apetalous artificial vascular of its blood capsule was to be fitted to thedescending aorta. The air vent pipe of its balloon capsule was to be connected to theIABP console. By the ECG trigger, synchronous charging-discharging gas was torealize volumetric displacement counterpulsation auxiliary. Implanting all sorts ofhemodynamic devices implanted into the chest through the left thoracic operation andligating the left descending arteries(LAD) and (or) cyclotron branches to establish theacute heart failure model.The application of physiological recorder was continuously togather the hemodynamic data before-after coronary artery ligation and before-aftercounterpulsation. Then evaluate the auxiliary effect of the two auxiliary devicesaccording to the difference of the data.Results9experimental heart failure model were got. The ligation of LAD and (or)cyclotron branches significantly affected the hemodynamic of the animals. Aftercoronary artery ligation cardiac output(CO) and mean arterial pressure (MAP)decreased by14.6%(P <0.02),22.7%(P <0.02) and LAP and LVP increased by35.7%(P <0.01),30.23%(P <0.01). It was validated that the branches of coronaryartery ligation can be used stably to establish a animal model of acute heart failure.After the auxiliary of IABP and the PACD,CO increased by5.3%and8.2%respectively, from4.06±0.24to4.06±1.06,4.62±0.86L/min (p <0.01). MAP increasedsignificantly by10.04%and15.84%, from80.02±0.34to80.02±2.36,95.47±5.38mmHg (p <0.05). SAP decreased by3.32%and7.82%, from87.00±12.82to 83.15±7.68,80.26±9.68mmHg (p <0.01). DAP decreased significantly by6.17%and10.39%, from62.56±8.79to56.63±11.42,52.63±12.39mmHg (p <0.01). LVDPdecreased by5.47%and12.25%, from75.76±0.27to75.76±1.39,63.28±2.7mmHg (p<0.01). Because of the auxiliary counterpulsation device, AABF-PABF increasedsignificantly by25.6%respectively (P <0.01),43.8%(P <0.01), from0.68±0.23to0.88±0.21,1.04±0.17(l/min),66.63±7.78mm (ml/min), MDAP increased by9.4%and12.6%respectively (P <0.01). After auxiliary PADP were90.06±11.32,97.25±13.67mmHg, significantly superior to SAP (p <0.01) before auxiliary.Conclusion Both the counterpulsation devices could reduce the afterload of leftventricular, improve diastolic arterial pressure, increase the coronary flow, improveforward blood flow, reduce myocardial oxygen consumption.They are effective to treatthe acute heart failure. Especially PACD auxiliary is much better. Aftercounterpulsation auxiliary SAP, DAP, LVDP were significantly deceased. IABP andPACD could significantly reduce the afterload of experimental animals with the acuteheart failure, especially PACD being superior to the former. After auxiliary CO, MAP,MDAP were significantly increased, and the PADP was significantly higher than SAPbefore auxiliary. IABP and the PACD could significantly improve cardiac function.And the effect of PACD was much better. After counterpulsation auxiliary AABF,PABF,MDAP, PADP significantly increased. So we can conclude that IABP andPACD are able to significantly improve the myocardial blood flow, improve myocardialperfusion, reduce myocardial oxygen consumption, and the PACD improve the forwardblood flow much better.
