ObjectiveTo investigate the clinical efficacy of Laparoscopic Radical Prostatectomy and Radical Retropubic Prostatectomy for T3prostate cancer.MethodsThe clinical data of34patients with T3prostate cancer who underwent radical prostatectomy from Mar.2010to Sep.2012were analyzed retrospectively. The indexes of operating time, blood loss, postoperative urinary continence rate and the change of Prostate Specific Antigen (PSA) were monitored.ResultsThe procedures of34cases, laparoscopic surgery in16cases and18cases of open surgery, were successfully operated besides one patient in LRP group transforming to open approach. There was no significant difference in operating time, blood loss, postoperative urinary continence rate and biochemicl recurrence rate. Hospital stay and gastrointestinal function recovery time of LRP group was shorter compared to RRP group. There were two cases with rectum injury; one case with anastomotic leakage of urine in LRP group; three months and six months latter, urinary continence rate of LRP and RRP group were81.3versus83.3%and87.5%versus88.9%. There was one case with urethral stricture in RRP group. There were respectively two cases in each group that PSA was up to more than0.2ng/ml.ConclusionFor T3Prostate cancer, compared with RRP, LRP achieved the same clinical efficacy with less trauma, quicker recovery, shorter hospitalization time. But LRP has higher rate of rectal injury than RRP. Effective control of bleeding and endoscopic suturing technique are the keys to the success of LRP.
