| Objectives:1) Via the epidemiological survey about some pilgrims in Gansu province,to find out the levels of Systolic blood pressure(SBP), Diastolic blood pressure(DBP) and Fasting plasma glucose(FPG);2) The prevalence status of hypertension,prehypertension, Diabetes mellitus(DM) and Impaired fasting glucose(IFG);3) The trends of SBP,DBP,FPG and the detection rate of hypertension and DM in three consecutive years;4) Analysis of the relations between blood cell parameters and hypertension,DM respectively,and explore the significance of blood cell parameters to prevention and treatment of hypertension and DM.Methods:as the pilgrims of Gansu for the study population,take some muslim pilgrims as research object,who do medical examination in Healthcare Center of Gansu Entry-Exit Inspection And Quarantine Bureau in2010,2011,2012three consecutive years,6081cases were analyzed after excluding incomplete information cases.All objects were collected fasting blood in moring after12hours fasting. Hitachi7080automatic biochemical analyzer was used to determine the fasting blood glucose(glucose oxidase),the blood cell parameters were determined by Sysmex XT-1800i automated hematology analyzer. Desktop sphygmomano-meter was used to measure blood pressure of right upper arm,and measured three times to take the averages. The mean were compared by two independent samples t-test and One-way ANOVA, linear regression analysis was used to reflect the trends of mean; The rate were compared by chi-square test, chi-square test for trend was used to reflect the trends of rate, linear regression analysis was used to reflect the relationship between blood cell parameters and SBP,DBP,FGP.Results:1.The levels of SBP,DBP and FPG of Gansu pilgrims:The mean level of SBP was141.01mmHg(141.87mmHg for male,139.85mmHg for female), The mean level of DBP was84.58mmHg(85.65mmHg for male,83.15mmHg for female), The mean level of FPG was5.80mmol/L(5.85mmol/L for male,5.74mmol/L for female).The mean levles of SBP,DBP and FPG were all increased with age.2.The detection rate of hypertension, prehypertension,DM and IFG:The detection rate of hypertension was59.84%(61.19%for male,57.38%for female), the detection rate of prehypertension was28.15%(28.11%for male,28.21%for female); The detection rate of DM was12.96%(14.03%for male,11.54%for female), the detection rate of IFG was15.29%(14.20%for male,16.75%for female).The detection rate of hypertension, DM and IFG were all increased with age,but the detection rate of prehypertension was decreased with age.The detection rate of hypertension has a more substantial accelerated process in age of39-47,analogous, DM’s more substantial accelerated process was in age of39～44.The detection rate of stage1hypertension, stage2hypertension and stage3hypertension were40.50%、14.09%、5.25%respectively,stage1and stage2hypertension were the mian hypertension type. The detection rate of ISH was26.89%(25.23%for male,29.09%for female),and it was increased with age.3.The SBP level of our target population was decreased slightly in2010,2011,2012three consecutive years,while trend of DBP level was no statistically significant.The male’s detection rate of hypertension was decreased slightly in three consecutive years,while there was no statistically significant in female.Both the FPG level and the detection of DM were increased significantly in three consecutive years.4.Red blood cell count(RBC), hematocrit(HCT), hemoglobin(Hb),white blood cell count (WBC), neutrophil cell absolute value, intermediate cell absolute value(MONO), platelet count(PLT) and Platelet distribution width(PDW) in the prehypertension group and hyperten-sion group were both higher than those in the normal group(P<0.05),while the mean corpuscular volume(MCV) was lower than that in the normal group(P<0.05); The Mean platelet volume(MPV) and Large platelet ratio(P-LCR) in the hypertension group were higher than those in the normal group(P<0.05); The RBC,WBC,NE,MONO in the hypertension group were higher than those in the prehypertension group(P<0.05),other Parametera was no statistically significant difference between the prehypertension group and hypertension group.There were relationship between SBP and RBC,MCV,RDW,HCT,Hb,WBC,NE,MONO and LYM(P<0.01),DBP had correlation with RBC, MCV, HCT, Hb, WBC, NE, MONO, LYM (P<0.01).5. MCV,RDW,RBC,NE,PDW in the prehypertension group and hypertension group were both higher than those in the normal group(P<0.05),while RBC was lower than that in the normal group(P<0.05); HCT, MPV,P-LCR in the prehypertension group was higher than those in the normal group(P<0.05),while PLT was lower than that in the normal group(P<0.05);Hb in the IFG group was lower than those in the normal group(P<0.05); Hb in the DM group was higher than those in the IFG group(P<0.05),while RBC in the DM group was lower than those in the IFG group,other Parametera was no statistically significant difference between the DM group and IFG group.There were correlation between FPG and RBC,MCV,WBC,NE and PDW (P<0.01).Conclusions:1.Compared with most of research results, Gansu pilgrims’mean levels of SBP,DBP and FPG are all at a high level,and the detection rates of hypertension and DM are at a high bit.lt should be attached great importance to pilgrims themselves and medical workers,and relevant departments should to strengthen publicity,education,Prevention and control work about hypertension and DM of Gansu pilgrims.Under the premise of respect for their Muslim doctrine, lower their blood pressure and blood sugar levels by lifestyle changes and reasonable diet,in this way,it can avoid or reduce the harm to the health by hypertension and DM of this population.in addition, according to the present study of result,we recommend to increase the screening efforts about hypertension and DM of Gansu pilgrims who are39to47years old and39to44years old respectively.2.There were number and form changes in peripheral blood cell form the patients with hypertension and DM,which may be related to the onset of acute and chronic complications (especially vascular complications) in the hypertension progress.Blood cell parameters have some significance for the prevention and treatment of hypertension:since most of the blood cell parameters were changed significantly during the period of prehypertension and IFG,so if we monitor the indicators of blood cell parameters early during the period of prehypertension and IFG,and give the patients positive prevention and treatment mearsures,it may have positive efferts on reducing the vascular complications caused by hypertension or DM. |
