Research BackgroundNosocomial infection has become a global threat to human health. The World Health Organization has attached great importance to this problem. It recommends five types of nosocomial infection control measures to the global: disinfection, isolation, aseptic operation, rational use of antimicrobial agents, and monitoring. It also recommends using monitoring to evaluate the nosocomial infection control effect. All of the five aspects are closely involved in clinical nursing work, throughout the whole process. Research results at home and abroad showed that30%-50%of the nosocomial infection is associated with inappropriate nursing action and invalid nursing management. Inappropriate nursing action is an important link in nosocomial infection formation. Therefore, to control nosocomial infection, nursing work is an important section to start with and to be paid great importance on. How to improve the prevention and control sense of nurses, and run through prevention and control work of nosocomial infection during nursing, make the nursing staff as the main force of prevention and control of nosocomial infection, are urgent subjects need to be studied.Research PurposeThe study established a nosocomial infection prevention and control management mode of "one committee, one people, three organizations" in the nursing team in a tertiary general hospital. Based on this, the nosocomial infection control committee and the groups formed a complete monitoring management chain of nursing nosocomial infection. This pattern was put into practice among the nursing team. Then, we study the effect of this model on improving nurses’ nosocomial infection prevention and control awareness. Thus, we can make nosocomial infection risk factors under control, and bring down its incidence rate.Materials and MethodsThis study adopts cluster random sampling method to select sample. We practiced nosocomial infection knowledge intervention to nueses during2009,2010,2011, and make surveys by each end of year. Every year,300nurses (60for each of the medical department, surgical department, gynecology department, pediatrics department, traditional Chinese medicine department, and other departments) from a certain municipal general hospital were selected randomly as study object."Nosocomial infection knowledge survey scale" was adopted to collect data. We also collect data from information department, medical record department, and nosocomial infection department. Data were typed into Access database twice, and processed through SPSS17.0statistical analysis software.(1) Value data were described with mean value and standard deviation, and analyzed with chi-square test to show the difference.(2) Enumeration data were described with rate (constituent ratio) and analyzed with F test to show the difference.Main ResultsThrough execution of "one committee, one people, three organizations" institution, establishment and improvement of nursing nosocomial infection management system, general training on nursing personnel, nurses’ nosocomial infection knowledge and skills have been greatly improved. After three years of comprehensive intervention, all of the fundamental quality, link quality, and final quality of nosocomial infection nursing have been getting better. Risk factors of nosocomial infection have been gradually prevented and controlled.Fundamental quality:Nurses have get better nosocomial infection knowledge, more operation skills, and enhanced self-protection awareness. Air and surface disinfection effect in key links and key department have been greatly improved. Operation skills in disinfection supply center have been more normative.Link quality:Compliance rate and qualification rate of hand washing in nurses have increased. Sterile nursing operation and the use of disposable medical articles of clinical nurses have been more valid. Infection rates of ICU indwelling catheter such as vascular catheter, urinary catheter, ventilator-associated catheter have diseased year by year.Final quality:Analysis on discharge record nosocomial infection data in the three years shows that nosocomial infection rates, prevalence rates, occupational injury of nurses have all decreased.Countermeasures and SuggestionsBased on the results, we provide the following results:Ideologically, managers should give priority to prevention and control work of nosocomial infection, nurses should renew their concept and enhance their alertness on nosocomial infection. Institutionally, nosocomial infection management should have clear-cut job responsibility and perfect institution, to strictly check nosocomial infection related indexes, and supervise nurses’ operation skills. During work, nursing personnel should perform nosocomial infection knowledge and skills skillfully, follow each rules and regulations about nosocomial infection management, and grasp nursing operation process exactly.All in all, nosocomial infection management work of nursing links should be persistent, without retreat or out of shape. Every system, standard, and form should be carried out seriously. We also should put this way of doing things as a habit, put nosocomial infection zero tolerance as a spirit. Thus, nosocomial infection rate can be controlled, and medical quality and patient safety can be better assured.
