| ObjectiveDouble steel plate through the analysis of the case review andgram needle treatment the curative effect difference betweenhumerus condyle fracture, evaluate the curative effect of twomethods of treatment, discusses between the humerus condylefracture of the surgical approach, choice of internal fixation,the prevention of complications, postoperative function exerciseand related issues.MethodsSelection in May2011-May2013wuhan PuAi (orthopaedichospital in wuhan city) hand outside of hospital bed of58patientswith fracture of the humerus condyle C1type of17cases,30casesof type C2C3type11cases,70, an average including30casesof male, female,28cases patients aged19of41.5years.According to the treatment of the needle was divided into doublesteel group and the group, double steel group29cases (158typeC1, C2, C3type6cases), gram needle group29cases (C1type9cases, C2type15cases, C3type5cases), were retrospectively analyzed the clinical curative effect of two internal fixationmethods, application SPSS17.0statistical software forstatistical analysis.ResultsAll patients were followed up for12months postoperatively,29cases of double steel plate group, all patients withpostoperative X-ray shows anatomical repositioning,1caseappeared mild postoperative oozing swelling of the wound healingdelay3-5days, the rest of the patients are all class a healingof incision, no myositis ossificans, ulnar neuritis, etc, allfractures healed.29cases gram needle group taken postoperativeX-ray anatomical repositioning,2cases with joint surface isuneven, class a healing of incision, in addition to a case of amerger between ulnar radial fracture patients with humerus condyleappear delay healing, healing on schedule. According to the improvement of Cassebaum criteria for12months postoperatively in patients with two groups of elbowjoint function score and comparison, double steel group was89.7%,the gram needle group was65.5%, the elbow joint function recoveredbetter double steel plate group, P <0.05, the difference wasstatistically significant, as shown in table5; Compared two groups of fractures of the clinical healing time, postoperative P <0.05,double steel group of fracture healing faster, difference wasstatistically significant, as shown in table6.ConclusionA double plate fixation between gram needle fixation treatmentof humerus condyle fracture has a better clinical therapeuticeffect, the former can improve the postoperative elbow range,promote the elbow joint function recovery, shorten the clinicalhealing time. Through this study, further confirmed that the ulnaolecranon v-shaped bone cutting into the road is the treatment ofhumerus condyle fracture between good surgical approach, which hasappeared full, easy operation, avoid damage triceps, conducive toelbow early function exercise, and other advantages. Intraoperative do fracture anatomical reduction, strongfixation, gentle operation, avoid excessive force, its necessaryto ulnar nerve forward; In under the guidance of earlypostoperative functional exercise, preventive use of nonsteroidalanti-inflammatory analgesics can reduce traumatic arthritis,myositis ossificans, rigidity, ulnar nerve paralysis and othercomplications. Anyhow, fractures open reduction, firm fixation andearly functional exercise is still the preferred method for thetreatment of between adult humerus condyle fracture. |
